ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
Assists in the day-to-day operations of the CHCS program by providing community health nursing services in the Del Norte County service area. May assist in the Humboldt County area when needed.
Visits homes to determine client and family needs, develops plan to meet needs, and provides needed community nursing services.
Instructs family in care and rehabilitation of client, and in maintenance of health and prevention of disease for family members.
Gives treatments to client following designated Health Care Provider’s instructions.
Assists community members and health field personnel to assess, plan for, and provide needed health and related services.
Investigates and intervenes in outbreaks of communicable disease within the community.
Refers clients with social and emotional problems to other UIHS departments and appropriate community agencies for assistance.
Teaches home nursing, maternal and childcare, and other subjects related to individual and community welfare.
Participates in programs to safeguard health of children, including child health conferences, school health, and group instruction for parents, immunization, and injury prevention programs.
Participates in the American Indian Infant Health Initiative (AIIHI) program and activities.
Works in collaboration to provide a quality immunization program for UIHS clients.

Assists in preparation of special studies and in research programs.
Assists in the applications for grant/funding and assists with the completion of required reports needed by grant/funding agency.
Will work with the Community Health Representatives (CHRs) to develop, implement, and evaluation care plans for clients.
Provides on-going feedback to the health care provider regarding patient response to care plans. Collaborates with the health care provider on determining changes in therapy if current therapy is deemed ineffective/inappropriate for the patient.
Participates in case management activities as assigned.
Documents care and communicate appropriately with other UIHS staff.
Records client responses to illness and treatment in the electronic health record and other appropriate records of care. Documents all client related activity according to section policy and procedure.
Attends department meetings and other assigned meetings.
Assists with the development of quality improvement activities and assists with completing required reports and program reviews. Adheres to accreditation and compliance standards/guidelines.

Maintains a professional, organized, and clean working environment by following organizational policies, guidelines, and safety standards.
Assists in development of department policy, procedures, and quality improvement activities within the clinic as directed.
SUPERVISORY RESPONSIBILITIES: This position will assist the Public Health Nurse Manager (PHNM) with the supervision of the Community Health Representatives (CHRs) and may provide supervision during the absence of the PHNM.
