Humboldt NeuroHealth’s WiredTogether Wraparound Program is seeking to hire Wraparound Clinical Case Manager to guide adoptive families through the Wraparound process. The successful candidate will have a heart for people and embody genuine caring for children and families facing adversity who are in need of support in order to remain intact as a family unit. This is a full time position based in Fortuna, CA. Starting pay will be $22-$26/hour with a benefits package.

Under the direct supervision of the Program Director, the Clinical Case Manager coordinates and facilitates the Child and Family Teams (CTFs), leads the development and implementation of individualized treatment and service plans, and provide direct services to children and families. The Clinical Case Manager serves as role model for the wraparound process, demonstrating particular commitment to family safety, unconditional care, family voice and choice, team-driven decision-making, strength-based planning and interventions, flexible services and supports, and community partnership and collaboration.

ESSENTIAL RESPONSIBILITIES AND DUTIES:
● Responsible for planning, coordinating, and facilitating regular Child and Family Teams (CFT) and weekly contact with clients in their homes and/or in the community, maximizing the involvement of natural team members and system partners.
● Provides leadership in the overall assessment, development, implementation and evaluation of individualized wraparound plans. Assumes responsibility for the implementation of plans, case management, including related activities and service delivery.
● Documents the plan development process by recording and distributing minutes of CFT meetings, writing progress notes, and updating Individualized Family Plans, Safety Plans, and planning documentation under the wraparound model.
● Prepares Individualized Family Plans and Safety Plans and coordinates each plan’s implementation, encouraging the maximum use of natural resources and community based services, and supplementing these with flexible wraparound staffing and funding
● Performs case management duties including treatment plan monitoring, advocacy, referral and linkage to other needed services and crisis interventions.
● Collaborates with the child, parents, teachers, doctors, etc. to maintain an interdisciplinary team approach to care.
● In consultation with the Program Director, coordinates both internal and external services in support of meeting identified family needs within budgetary, operational and ethical constraints.
● Demonstrate the ability to initiate, build, and maintain relationships based on trust and respect; work with various cultural and ethnic clients, employees and families in a tactful and effective manner.
● Completes all required documentation according to Agency and program standards within required time constraints; writes clear, accurate and timely progress notes consistent with each contact/visit.
● Provides crisis stabilization to child/family teams when plans disrupt and/or crisis situations occur.
● Meets Agency service percentage standard reflecting expected time spent with clients.
● Attends weekly staff meetings, supervision and case management staff meetings as scheduled.
● Other duties as assigned.

Minimum Requirements:
-Possess a Master’s or Doctoral degree in Social Work, Psychology, Counseling, or Marriage and Family therapy, and registered or eligible to register with the California Board of Behavioral Sciences.
