08. Proper hot and cold holding temperatures.

Conditions Observed: Measured half and half 46F and ambient air 45F in Emerson refrigerator behind the bar. Adjust/service/repair or replace this refrigerator with a commercial unit capable of holding cold potentially hazardous foods at or below 41F at all times.

ROACHY SAYS:

“Except during preparation, cooking, cooling, transportation to or from a retail food facility for a period of less than thirty minutes, when time is used as the public health control, or as otherwise specified, potentially hazardous foods shall be maintained at or below 41F or at or above 135F. The following foods may be held at or below 45F: raw shell eggs, unshucked live molluscan shellfish, pasteurized milk and pasteurized milk products in original sealed containers, potentially hazardous foods held during transportation. Potentially hazardous foods held for dispensing in serving lines and salad bars may be held at 45F, not to exceed 12 hours in a 24 hour period, if unused portions are discarded. 10. Proper cooling methods.

Conditions Observed: Measured bean and ham soup 53F edge and 57F center in Atosa upright refrigerator. Per employees, the soup had been cooling since last night. Potentially hazardous foods must be rapidly cooled from 135F - 70F within 2 hours and from 70F - 41F within 4 hours. Use the food cooling log to document the cooling process to ensure time and temperature parameters are always being met. Approximately 7 quarts of improperly cooled soup was discarded, this major violation was corrected.

ROACHY SAYS:

“After heating, potentially hazardous foods shall be rapidly cooled from 135F to 70F within 2 hours, and then from 70F to 41F within 4 hours. Conditions Observed: Observed clean glasses stored on soiled cloth towels in bar refrigerator. Glasses must be stored on a surface that is smooth, durable, nonabsorbent and easy to clean. Discontinue storing glasses on cloth towels to prevent cross-contamination.

Food-contact surfaces and multiservice utensils shall be effectively washed, rinsed, and sanitized to remove or completely loosen soils by use of manual or mechanical methods. Precleaning may be required. Thermometers shall be calibrated in accordance with manufacturer's specifications as necessary to ensure their accuracy. Conditions Observed: Observed floors and coving have failed behind the bar. Observed several cracked tiles and missing grout in kitchen floors. Replace floors and/or grout to provide a surface that is smooth, durable, nonabsorbent and easy to clean.

Conditions Observed: Measured chlorine sanitizer 0PPM at kitchen dishwasher. Use chlorine test strips daily to verify the dishwasher is sanitizing at the correct concentration of 50PPM. Employee primed sanitizer line and chlorine sanitizer was provided at 50PPM. Thank you for correcting this major violation.

Observed plastic container of sliced yellow cheese stored on top of an uncovered container of sliced white cheese in cookline ice bath. Do not stack inserts as uncovered foods underneath may become contaminated and top insert is out of contact with ice bath and may result in temperature violations. Observed open bags of flour and pasta on shelf adjacent to cookline. Cover all stored food to prevent contamination and potential vermin attraction. Observed raw poultry stored over raw ground beef in Maximum 2 door refrigerator. Observed raw shell eggs stored over smoked ham in Arctic Air refrigerator. Store raw potentially hazardous foods from animal origin based on final internal cooking temperatures to prevent cross-contamination. Ready-to-eat foods shall be stored over raw potentially hazardous foods. Use refrigeration food storage order handout that was provided for guidance.

Remove this container from premises and provide a sanitary container that shall be cleaned and sanitized after each use to prevent buildup.

Thermometers shall be calibrated in accordance with manufacturer's specifications as necessary to ensure their accuracy. 45. Floor, walls, and ceilings: built, maintained, clean.

Conditions Observed: Observed standing water and grease accumulation on floor under cookline equipment. Find source of standing water and repair, if applicable. Clean floors regularly to prevent buildup. Observed grease and/or food debris on kitchen walls and ceilings. Clean impacted areas to prevent buildup. Observed floors and coving have failed behind the bar. Replace floors to provide a surface that is smooth, durable, nonabsorbent and easy to clean with 3/8” radius coving that extends at least 4 inches up the wall. Submit manufacturer specification sheets or floor samples to this office for approval prior to purchase and installation.

ROACHY SAYS:

“Food facilities shall be fully enclosed in a building consisting of permanent floors, walls, and an overhead structure that meets minimum standards. The walls and ceiling shall be smooth, durable, nonabsorbent, and easily cleanable. Flooring and base coving shall be smooth, durable, and made of approved nonabsorbent material that is easily cleanable. Floor surfaces shall be coved at the juncture of the floor and wall with a 3/8 inch minimum radius and shall extend up the wall at least 4 inches. (114143(d), 114266, 114268, 114268.1, 114271, 114272)”

Routine Inspection
07. Adequate handwash facilities supplied and accessible.

Conditions Observed: The bar is lacking required supplies, in dispensers, for handwashing. Provide pump soap and single-use paper towels, in dispensers, behind the bar at all times for correct handwashing.

ROACHY SAYS:

“Handwashing facilities shall be provided within or adjacent to toilet rooms, food preparation, and warewashing areas. Handwashing facilities shall be clean, unobstructed, and accessible at all times. Handwashing facilities shall be provided with soap and sanitary single use towels in approved dispensers. Dispensers shall be maintained in good repair. A handwashing facility shall not be used for purposes other than handwashing. (113953, 113953.1, 113953.2, 114067(f))”

08. Proper hot and cold holding temperatures.

Conditions Observed: Measured sliced mushrooms 44.5F and shredded lettuce 42.5F with probe thermometer in ice baths on the cookline. Observed plastic containers being used in ice baths. Recommend using metal containers, with containers fully submerged in ice to the top of the food, to help conduct cooler temperatures to hold cold potentially hazardous food at or below 41F at all times.

ROACHY SAYS:

“Except during preparation, cooking, cooling, transportation to or from a retail food facility for a period of less than thirty minutes, when time is used as the public health control, or as otherwise specified, potentially hazardous foods shall be maintained at or below 41F or at or above 135F. The following foods may be held at or below 45F: raw shell eggs, unshucked live molluscan shellfish, pasteurized milk and pasteurized milk products in original sealed containers, potentially hazardous foods held during transportation. Potentially hazardous foods held for dispensing in serving lines and salad bars may be held at 45F, not to exceed 12 hours in a 24 hour period, if unused portions are discarded. Food preparation shall not exceed two cumulative hours without a return to the required holding temperatures. (113996, 113998, 114037, 114343(a))”

20. Consumer advisory provided for raw or undercooked foods.

Conditions Observed: Facility serves raw or undercooked food (hamburgers) and fails to inform consumers of the significantly increased risk of consuming those foods by way of a disclosure and reminder. Identify the animal derived foods by marking an asterisk on the menu that denotes a footnote that includes the following disclosure statement, “Consuming raw or undercooked meats, poultry, seafood, shellfish, or eggs may increase your risk of foodborne illness, especially if you have certain medical conditions.”

ROACHY SAYS:

“If a food of animal origin is served raw, undercooked, or without otherwise being processed to eliminate pathogens, either in ready-to-eat form or as an ingredient in another ready-to-eat food, the permitholder shall inform consumers of the significantly increased risk of consuming those foods by way of a disclosure pursuant to subdivision (b) of 114093 and reminder pursuant to subdivision (c) of 114093, using brochures, deli case or menu advisories, label statements, table tents, placards, or other effective written means. (114012, 114093)”

28. Food separated and protected.

Conditions Observed: Observed bagged croutons stored under chemicals on same shelving unit. Find another location for crouton storage to protect from possible contamination from chemicals stored above.

ROACHY SAYS:

“Food shall be protected from contamination during transportation, storage, preparation, holding, and display. Adequate and suitable counter space shall be provided for all food preparation. Food preparation shall be conducted wthin an approved food compartment or fully enclosed food facility. Open-air barbecues shall be separated from public access. (113984(a-f), 113986, 114060, 114067(a,d,e,j), 114069(a, c, d), 114077, 114089.1(c), 114143(c,e))”

34. Non-food-contact surfaces clean.

Conditions Observed: Observed interior and exterior of equipment throughout facility (microwave, refrigerators, stove) is visibly soiled with food debris. Clean non-food contact surfaces of equipment regularly to prevent buildup. Observed container used as a double boiler to reheat soup with excessive grease accumulation and food debris. Remove this container from premises and provide a sanitary container that shall be clean and sanitized after each use to prevent buildup.

ROACHY SAYS:

“Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris. (114115(b))”

36. Equipment / Utensils approved; installed, clean, good repair, capacity.

Conditions Observed: Observed mold-like substance in soda gun holsters at the bar. The holsters are usually equipped with a drain tube that shall be indirectly plumbed to the sanitary sewer by way of an air gap through a floor sink or other approved receptor, however the soda gun holsters are lacking drain tubes allowing waste to pool and create unsanitary conditions (mold). Ask your soda supplier to replace the soda gun holsters with drain tubes. Clean and sanitize holsters regularly to prevent mold growth. Observed non-commercial equipment in use at facility (microwave, stove, mini refrigerators). Replace with commercial equipment (NSF, UL, ANSI, etc.). Observed mold-like substance on glass racks. Clean and sanitize glass racks to remove mold, or replace if applicable.

ROACHY SAYS:

“Utensils and equipment shall be kept fully operative and in good repair. All new and replacement equipment shall be properly installed and be certified or classified for sanitation by an ANSI accredited certification program. Electrical appliances must meet UL standards. Single-use articles shall not allow migration of deleterious substances or impart colors, odors, or tastes to food. Multi-use food contact surfaces must be smooth, free of breaks, chips, and similar imperfections; free of sharp internal angles, corners, and crevices; finished to have smooth welds and joints; be accessible for cleaning and inspection. Unless specified, wood and wood wicker may not be used as a food-contact surface. Unless specified, copper and copper alloys such as brass may not be used in contact with a food that has a pH below six or for a fitting between a backflow preventer and a carbonator. Beverage tubing and cold-plate beverage cooling devices cannot be installed in contact with stored ice intended to be used as food. Equipment that is not movable must be installed to allow for cleaning in place. Cutting or piercing parts of can openers must be readily removable. Facilities that wash, rinse, soak, thaw, or similarly prepare foods shall be provided with a approved food preparation sink. Reservoirs that supply water to produce foggers must be cleaned and maintained. Surfaces such as cutting blocks and boards that are subject to scratching and scoring shall be resurfaced or replaced if they can no longer be effectively cleaned and sanitized. 37. Equipment, utensils and linens: storage and use.

Conditions Observed: Observed ice scoop stored in the ice in ice machine. Store ice scoop in a sanitary location to prevent cross-contamination of ice. Observed clean utensils stored in visibly soiled space on cookline. Clean and sanitize impacted area regularly to protect clean utensils from contamination, or store clean utensils in another sanitary location.

ROACHY SAYS:

“Clean equipment, utensils, linens, and single-use articles shall be stored at least six inches above the floor in an approved location within the fully enclosed permitted food facility. Clean equipment, utensils, linens, and single-use articles shall not be exposed to splash, dust, vermin, or other forms of contamination. Non-food items shall be stored and displayed separate from food and food-contact surfaces. During pauses in food preparation utensils shall be stored with handles above the top of the food, on a clean sanitized surface, in running water, or in water that is at least 135øF. Single-use articles and multiservice utensils shall be handled, displayed, and dispensed so that contamination of food and lip-contact surfaces is prevented. Extra preset tableware shall be removed when a consumer is seated or cleaned and sanitized before further use. Pressurized cylinders shall be securely fastened to rigid structure. Linens must be free of food residue and soil and be laundered as required. 39. Thermometers provided and accurate.

Conditions Observed: Observed Arctic Air refrigerator, holding potentially hazardous foods, lacking a thermometer. Obtain a thermometer and install in the warmest part of the refrigerator, in a location that is readily visible to monitor the temperature regularly.

ROACHY SAYS:

“An accurate easily readable metal probe thermometer suitable for measuring temperature of food shall be available to the food handler.
