Remove unused and nonfunctional equipment from the premises. Observed ice buildup on bottom of True 2 door freezer. Maintain equipment clean and in good repair.

Conditions Observed: Facility has remodeled (removed middle island counter, installed new counter tops and cabinets, relocated hot food cases and beverage/coffee equipment, removed front handwash sink). An existing facility that plans to remodel shall submit plans to this office for review and approval prior to purchasing and installing equipment, or making any changes to the existing food facility.

Conditions Observed: Measured nacho cheese 130F with probe thermometer from the nacho cheese dispenser. Nacho cheese is a potentially hazardous food that must be held at or above 135F at all times. Operator discarded nacho cheese, thank you for correcting this major violation.

“Except during preparation, cooking, cooling, transportation to or from a retail food facility for a period of less than thirty minutes, when time is used as the public health control, or as otherwise specified, potentially hazardous foods shall be maintained at or below 41F or at or above 135F. The following foods may be held at or below 45F: raw shell eggs, unshucked live molluscan shellfish, pasteurized milk and pasteurized milk products in original sealed containers, potentially hazardous foods held during transportation. Potentially hazardous foods held for dispensing in serving lines and salad bars may be held at 45F, not to exceed 12 hours in a 24 hour period, if unused portions are discarded. Conditions Observed: Observed excessive buildup on nacho cheese dispenser. This is a food contact surface that must be washed, rinsed and sanitized at least every 4 hours to prevent cross-contamination. Due to the nacho cheese dispenser holding temperature at 130F, the nacho cheese dispenser was retired for the day. Wash, rinse and sanitize this piece of equipment prior to putting it back into service. This major violation was corrected on site, thank you.

Conditions Observed: Observed nacho cheese bag in nacho cheese dispenser lacking a date code. Per manufacturer, the nacho cheese is only good for 5 days after it has been opened and put into the warmer. You are required to date the nacho cheese bag for 5 days, once opened. See California Department of Public Health Memorandum regarding nacho cheese warming units at retail locations. Operator removed nacho cheese bag from the dispenser. Observed nacho cheese was discolored and not dated. Operator discarded nacho cheese bag, thank you.

Conditions Observed: Measured grilled chicken burrito 130.5F with probe thermometer in Aztec Grill hot hold unit. Observed the chicken burrito was stored on the edge of the rack, not in direct contact with heat source. Ensure burritos are in direct contact with heat source at all times. The burrito was discarded, thank you. Measured several items in new deli reach-in “island” refrigerator above 41F (chopped cantaloupe: 50F; chopped honeydew: 53F; tuna salad 43F; krispy chicken wrap: 42; wedge sandwich 48.5; BBQ chicken salad: 47.5F; chicken tenders 48F). Adjust/repair/replace this refrigerator to hold cold potentially hazardous foods at or below 41F at all times. Install readily visible thermometers in the front of top and middle shelf to monitor holding temperatures at all times. The cantaloupe and honeydew were discarded and all other foods were moved to the walk-in refrigerator holding at 41F. This is a major violation that will require a reinspection. Contact this office to schedule reinspection once this refrigerator has been adjusted or repaired.

Observed mold-like substance in interior of ice machine. Clean and sanitize ice machine per manufacturer’s instructions on inside panel. Keep this piece of equipment on a regular cleaning and maintenance schedule to prevent mold growth.

ROACHY SAYS:

“Utensils and equipment shall be kept fully operative and in good repair. All new and replacement equipment shall be properly installed and be certified or classified for sanitation by an ANSI accredited certification program. Electrical appliances must meet UL standards. Single-use articles shall not allow migration of deleterious substances or impart colors, odors, or tastes to food. Multi-use food contact surfaces must be smooth, free of breaks, chips, and similar imperfections; free of sharp internal angles, corners, and crevices; finished to have smooth welds and joints; be accessible for cleaning and inspection. Unless specified, wood and wood wicker may not be used as a food-contact surface. Unless specified, copper and copper alloys such as brass may not be used in contact with a food that has a pH below six or for a fitting between a backflow preventer and a carbonator. Beverage tubing and cold-plate beverage cooling devices cannot be installed in contact with stored ice intended to be used as food. Equipment that is not movable must be installed to allow for cleaning in place. Cutting or piercing parts of can openers must be readily removable. Facilities that wash, rinse, soak, thaw, or similarly prepare foods shall be provided with a approved food preparation sink. Reservoirs that supply water to produce foggers must be cleaned and maintained. Surfaces such as cutting blocks and boards that are subject to scratching and scoring shall be resurfaced or replaced if they can no longer be effectively cleaned and sanitized. 38. Adequate ventilation and lighting, designated areas, use.

Conditions Observed: Observed non-functioning lights in the following areas: -back room in front of walk-in refrigerator -one restroom Repair/replace light bulbs to provide adequate lighting.

ROACHY SAYS:

“Approved exhaust hoods shall be provided to remove toxic gases, heat, grease, vapors and smoke. Canopy-type hoods shall extend six inches beyond all cooking equipment. Toilet rooms shall be consistent with local building codes and vented to the outside air by a screened openable window, air shaft, or light-switch activated exhaust fan. Adequate lighting shall be provided in all areas to facilitate cleaning and inspection. Light fixtures in areas where open food is stored, served, prepared, and where utensils are washed shall be of shatterproof construction or protected with light shields. 45. Floor, walls, and ceilings: built, maintained, clean.

Conditions Observed: Observed floor is failing in front of walk-in refrigerator. Repair/replace floor to provide a surface that is smooth, durable, nonabsorbent, and easy to clean. Submit floor samples and/or manufacturer specification sheets to this office for approval prior to purchase and installation.

ROACHY SAYS:

“Food facilities shall be fully enclosed in a building consisting of permanent floors, walls, and an overhead structure that meets minimum standards. The walls and ceiling shall be smooth, durable, nonabsorbent, and easily cleanable. Flooring and base coving shall be smooth, durable, and made of approved nonabsorbent material that is easily cleanable. Floor surfaces shall be coved at the juncture of the floor and wall with a 3/8 inch minimum radius and shall extend up the wall at least 4 inches. (114143(d), 114266, 114268, 114268.1, 114271, 114272)”

50. Valid Facility Permit: expiration date, proper operation category.

Conditions Observed: The facility is currently chopping onions and holding them under temperature control for use as a condiment for hot dogs. The facility is currently operating under a minimal prep food permit, which does not include this practice. Immediately stop chopping onions and cold holding them for use as condiments on hot dogs. You may purchase pre-chopped onions and transfer them to the cold holding unit, or you may upgrade to a full-prep food permit if you choose to continue this practice. Contact this office for more information, if necessary.

ROACHY SAYS:
