Routine Inspection
15. Food contact surfaces: clean and sanitized.

Conditions Observed: Measured chlorine sanitizer 0PPM at bar dishwasher. Adjust/service/repair bar dishwasher to deliver chlorine sanitizer at 50PPM at all times. This is a major violation that requires a reinspection on or after 11/23/21, or you may provide proof of the correction to this office by 4PM on 11/22/21 in place of a reinspection.

ROACHY SAYS:

“Food-contact surfaces and utensils shall be clean to sight and touch. Food-contact surfaces and multiservice utensils shall be effectively washed, rinsed, and sanitized to remove or completely loosen soils by use of manual or mechanical methods. Precleaning may be required. Manual sanitization shall be accomplished during the final rinse by contact with solution containing 100 ppm available chlorine, 200 ppm quaternary ammonium, or another approved sanitizer. Mechanical sanitization shall be accomplished in the final rinse by achieving a utensil surface temperature of 160F or by contact with solution containing 50 ppm available chlorine, 200 ppm quaternary ammonium, or another approved sanitizer. After cleaning and sanitizing, equipment and utensils shall be air dried. Mechancial warewash machines must be approved and installed and operated according to manufacturer's specifications. Food contact surfaces, utensils, and equipment shall be cleaned and sanitized at the following times: before each use with different type of raw food of animal origin, when changing from raw food to ready to eat food, between uses with raw produce and potentially hazardous food, before using a thermometer, and any time during the operation when contamination may have occurred. Warewash sinks used to wash wiping cloths, wash produce, or thaw food must be cleaned and sanitized before and after these uses. Equipment, food contact surfaces, and utensils must be cleaned and sanitized throughout the day at least every four hours or as needed to prevent contamination. 22. Hot and Cold Water Available

Conditions Observed: Measured water 94F at nonadjustable handwashing sink. The nonadjustable handwashing sink is required to provide warm water between 100F & 108F. Adjust/repair nonadjustable handwashing sink to deliver warm water 100F - 108F at all times for handwashing.

ROACHY SAYS:

“An adequate, protected, pressurized, approved potable supply of hot and cold water shall be provided at all times. Hot water shall be supplied at a minimum temperature of 120F when measured from the faucet. The temperature of water provided to handwashing facilities shall be at least 100F. Handwashing facilities equipped with non-adjustable faucets shall have their temperature set between 100F and 108F. Hot water shall be provided at the food preparation sink and janitorial sink. 35. Warewashing: installed, maintained, used, Adequate Means to measure sanitizer

Conditions Observed: Facility is lacking thermal labels to verify the high temperature dishwasher is providing water hot enough for sanitizing. Obtain thermal labels, store in a readily available location, and use daily to verify the high temperature dishwasher is sanitizing properly.

ROACHY SAYS:

“Facilities that prepare food shall be equipped with a manual warewashing sink that has at least three compartments with two integral metal drainboards, except as specified in 114099(c). A warewashing sink shall not be used for handwashing except as specified in 114125(a). Sink compartments shall be large enough to accommodate immersion of the largest equipment and utensils used. Testing materials shall be provided to adequately measure the applicable sanitization method used during manual or mechanical warewashing. Conditions Observed: Measured roast beef 48F and sliced turkey 47F in top portion of sandwich/salad prep table. Measured pasta 43F in bottom portion (top right drawer) of cookline prep table. Adjust/service/repair refrigerator units to hold cold potentially hazardous foods at/below 41F at all times.

“Except during preparation, cooking, cooling, transportation to or from a retail food facility for a period of less than thirty minutes, when time is used as the public health control, or as otherwise specified, potentially hazardous foods shall be maintained at or below 41F or at or above 135F. The following foods may be held at or below 45F: raw shell eggs, unshucked live molluscan shellfish, pasteurized milk and pasteurized milk products in original sealed containers, potentially hazardous foods held during transportation. Potentially hazardous foods held for dispensing in serving lines and salad bars may be held at 45F, not to exceed 12 hours in a 24 hour period, if unused portions are discarded. 22. Hot and Cold Water Available

Conditions Observed: Non-adjustable handwash sink in server station is delivering water at 94F. Adjust non-adjustable handwash sink to provide water between 100F - 108F for proper handwashing.

24. No rodents, insects, birds, or animals.

Conditions Observed: Observed several dried rodent droppings in broom closet. Follow CDC guidelines, clean and sanitize impacted area and monitor for new rodent activity. Contact professional pest control to increase service if new rodent activity is observed.

“A food facility shall be kept free of vermin. Live animals are prohibited from food facilities except as specified in 114259.5. Food facility employees shall not care for or handle animals that may be present in the food facility. Food facility employees are required to wash their hands after handling or caring for fish in an aquarium and/or shellfish, or crustacea, in display tanks and as specified in 114259.4. 36. Equipment / Utensils approved; installed, clean, good repair, capacity.

Conditions Observed: Observed buildup on bottom interior of keg refrigerator behind the bar. Maintain equipment clean to prevent buildup or vermin attraction. Observed mold-like substance in ice machine. Clean and sanitize ice machine per manufacturer’s instructions to prevent mold growth.

“Utensils and equipment shall be kept fully operative and in good repair. All new and replacement equipment shall be properly installed and be certified or classified for sanitation by an ANSI accredited certification program. Electrical appliances must meet UL standards. Single-use articles shall not allow migration of deleterious substances or impart colors, odors, or tastes to food. Multi-use food contact surfaces must be smooth, free of breaks, chips, and similar imperfections; free of sharp internal angles, corners, and crevices; finished to have smooth welds and joints; be accessible for cleaning and inspection. Unless specified, wood and wood wicker may not be used as a food-contact surface. Unless specified, copper and copper alloys such as brass may not be used in contact with a food that has a pH below six or for a fitting between a backflow preventer and a carbonator. Beverage tubing and cold-plate beverage cooling devices cannot be installed in contact with stored ice intended to be used as food. Equipment that is not movable must be installed to allow for cleaning in place. Cutting or piercing parts of can openers must be readily removable. Conditions Observed: Measured cranberry juice and 1/2 & 1/2 44F with probe thermometer in under counter refrigerator behind the bar. Adjust/service/repair this unit to hold cold potentially hazardous foods at or below 41F at all times.

Conditions Observed: Observed standing liquid and mold-like substance in keg refrigerator behind the bar. Clean non-food contact surfaces of equipment regularly to prevent buildup.

“Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris. (114115(b))”

37. Equipment, utensils and linens: storage and use.

Conditions Observed: Observed utensils are stored in “dirt room” downstairs. Utensils must be stored in an approved location with floors, walls and ceilings that meet California Retail Food Code. Please move utensils to another location to prevent contamination. If you no longer use the utensils stored in this location, please remove them from the premises prior to your next routine inspection.

ROACHY SAYS:

“Clean equipment, utensils, linens, and single-use articles shall be stored at least six inches above the floor in an approved location within the fully enclosed permitted food facility. Clean equipment, utensils, linens, and single-use articles shall not be exposed to splash, dust, vermin, or other forms of contamination. Non-food items shall be stored and displayed separate from food and food-contact surfaces. During pauses in food preparation utensils shall be stored with handles above the top of the food, on a clean sanitized surface, in running water, or in water that is at least 135øF. Single-use articles and multiservice utensils shall be handled, displayed, and dispensed so that contamination of food and lip-contact surfaces is prevented. Extra preset tableware shall be removed when a consumer is seated or cleaned and sanitized before further use. Pressurized cylinders shall be securely fastened to rigid structure. Routine Inspection
01. Demonstration of knowledge; food safety certification.

Conditions Observed: Facility no longer has a Food Safety Manager (FSM). New chef is scheduled to take FSM class and exam this Summer. Once certificate is obtained, submit a copy to this office for verification by 8/25/19 to avoid a reinspection.

ROACHY SAYS:

“The Person In Charge (PIC) and all employees shall have adequate knowledge and shall be properly trained in food safety as it relates to their assigned duties. The PIC shall have adequate knowledge of major food allergens, foods identified as major food allergens, and the symptoms that a major food allergen could cause. The PIC means a designated person who has knowledge of safe food handling practices and the major food allergens as they relate to the specific food preparation activities that occur at the food facility. Conditions Observed: Observed bar lacking paper towels for handwashing. This is a repeat minor violation. Install a paper towel dispenser behind the bar and keep stocked at all times for proper handwashing.

Conditions Observed: Observed several small, dry rodent feces in broom closet. Observed 2 small, dry rodent feces on shelf in downstairs storage area. This is a repeat major violation. Remove all food and utensils from downstairs storage area, or put into rigid containers with tight fitting lids to prevent contamination from rodents. Clean and sanitize impacted areas and continuously monitor for new evidence of rodent activity. Increase pest control efforts if necessary. Seal holes around pipes in broom closet. This is a major violation that will require a reinspection on or after 7/2/19.

31. Food storage; food storage containers identified.

Conditions Observed: Observed buckets of food stored on the floor in walk-in refrigerator. Store all food at least 6 inches above the floor to prevent contamination and potential vermin attraction.

ROACHY SAYS:

“Adequate and suitable space shall be provided for the storage of food. Food shall be protected from contamination by storing the food in a clean, dry location, where it is not exposed to splash, dust, vermin, or other forms of contamination or adulteration, and at least six inches above the floor. Food shall not be stored in any of the following ways: in locker rooms, toilet rooms, dressing rooms, refuse rooms, mechanical rooms, under sewer lines that are not shielded to intercept potential drips, under leaking water lines, including leaking automatic fire sprinklers heads, or under lines on which water has condensated, under open stairwells or under sources of contamination. Working containers holding food or food ingredients that are removed from their original packages for use in the food facility shall be identified with the common name of the food. Non-prepackaged food may not be stored in direct contact with undrained ice. Products that are held by the permit holder for credit, redemption, or return to the distributor, such as damaged, spoiled, or recalled products shall be segregated and held in designated areas that are separated from food, equipment, utensils, linens, and single-use articles. 45. Floor, walls, and ceilings: built, maintained, clean.

Conditions Observed: Observed grease accumulation on kitchen floor under cookline equipment. Clean floors regularly to prevent buildup. Beverages are stored in downstairs storage area with a dirt floor. This is not an approved space for food storage. Immediately remove beverages from this area until a floor that is smooth, durable, nonabsorbent and easy to clean is provided.

ROACHY SAYS:

“Food facilities shall be fully enclosed in a building consisting of permanent floors, walls, and an overhead structure that meets minimum standards. The walls and ceiling shall be smooth, durable, nonabsorbent, and easily cleanable. Flooring and base coving shall be smooth, durable, and made of approved nonabsorbent material that is easily cleanable. Floor surfaces shall be coved at the juncture of the floor and wall with a 3/8 inch minimum radius and shall extend up the wall at least 4 inches. Conditions Observed: Observed beverages stored in downstairs storage area with a dirt floor. If you choose to use this space for food storage, it must conform to California Retail Food Code requirements. Provide a floor that is smooth, durable, nonabsorbent and easy to clean. Floor coving is not required in areas where food is stored only in unopened bottles, cans, cartons, sacks or other original shipping containers. Submit manufacturer specification sheets and/or floor samples to this office for approval prior to purchase and installation.

16. Food obtained from approved source.

Conditions Observed: Observed vacuum sealed meat labeled “ham” with sharpie in chest freezer. Per employee, the meat came from a member of the Ingomar Club. All food served from the permitted facility shall come from an approved source. All meat products shall be USDA grade meats. The food will be returned to the member and will not be served to customers. Observed facility is manufacturing ice cream without the required dairy license from California Department of Food and Agriculture. Immediately stop manufacturing ice cream until the required dairy license is obtained. Once obtained, submit copy to this office for verification prior to continuing this practice in the facility. In the meantime, obtain commercial manufactured ice cream from an approved source. Observed person in charge posted sign on ice cream maker that states, “do not use.” These are major violations that will require a reinspection on or after 1/29/19.

ROACHY SAYS:

“All food, including ice, shall be obtained from an approved source. (113980, 114021-114031, 114041(b), 114376, Humboldt County Code 512-4)”

19. Compliance with variance, specialized process, reduced O2 packaging, HACCP plan.

Conditions Observed: Observed vacuum sealer in use at facility and vacuum sealed meats, including seafood, without an approved HACCP plan from California Department of Public Health. Packaging potentially hazardous food products using a reduced-oxygen packaging method require an approved HACCP from CDPH as it creates the perfect environment for Clostridium botulinum, if temperature abused. Immediately stop using the vacuum sealer for reduced oxygen packaging until a HACCP plan is obtained by CDPH. Once obtained, submit a copy to this office for verification prior to using vacuum sealer in facility. In the meantime, use permeable food grade bags (Ziplocks) to store potentially hazardous foods. This is a major violation that will require a reinspection on or after 1/29/19. Observed person in charge posted sign on vacuum sealer that says, “do not use”.

ROACHY SAYS:

“A food facility may engage in the following activities only pursuant to a written and approved HACCP plan: smoking food as a method of preservation, curing food, using food additives or adding components such as vinegar as a method of food preservation or to render a food so that it is not potentially hazardous, operating a molluscan shelfish life support system display tank, custom processing of animals for personal use as food and not for sale or service in a food facility, using acidification or water activity to prevent the growth of Clostridium botulinum, packaging potentially hazardous food using a reduced oxygen packaging method except as specified in 114419.2(a - c), or preparing food by another method that is determined by the Enforcement Agency to require a HACCP plan. Food facilities with CDPH approved variances must maintain a copy at the food facility at all times. A variance may be suspended or revoked if the food facility is found to be non-complaint with the terms of the variance. Conditions Observed: Observed rodent feces on shelves in downstairs storage room. This is a repeat violation. Remove all bulk food storage from this area, as there is rodent activity. Discard all bags that are currently stored downstairs. Increase professional pest control service to identify and repair ingress holes and remove rodents from facility. This is a major violation that will require a reinspection on or after 1/29/19.
