Conditions Observed: Observed front handwash sink (adjacent to French fry hot hold), lacking paper towels in the paper towel dispenser. Keep all handwash stations fully stocked with required supplies in dispensers at all times for proper handwashing. Manager stocked paper towels, this minor violation was corrected.

Conditions Observed: Observed leak at dish sprayer faucet located at the 3 compartment sink and a leak at the faucet located at the janitorial sink. Repair leaks.

Conditions Observed: Observed employee use infrared thermometer to conduct a health assessment then return to work at drive-thru window without handwashing. Observed employee handle raw egg product, then use cooking utensil without handwashing. Handwashing must be conducted after handling raw potentially hazardous foods of animal origin, prior to conducting food prep, when changing tasks, prior to donning a new pair of gloves and anytime when hands may become contaminated. Person in charge instructed both employees to handwash and will hold a handwash training refresher course for all staff.

Conditions Observed: Observed food handler don blue gloves, handle raw beef patty then remove gloves before donning clear gloves and handle ready to eat foods; the handwashing step prior to donning clear gloves and handling ready to eat foods was omitted each time. Inspector discussed proper glove use and handwashing with food handler and observed correct subsequent handwashing. Please review handwashing and verify that all employees are properly trained in handwashing and glove use prior to handling foods and food contact surfaces.

Conditions Observed: Cooking oil was observed on floor in puddle of water. Store all foods at least 6 inches off of floor in a manner that minimizes potential for cross contamination. Boxes of pies and doughnut sticks were stored in walk in cooler without covers. Keep all foods covered to minimize risk of contamination.

“Adequate and suitable space shall be provided for the storage of food. Food shall be protected from contamination by storing the food in a clean, dry location, where it is not exposed to splash, dust, vermin, or other forms of contamination or adulteration, and at least six inches above the floor. Food shall not be stored in any of the following ways: in locker rooms, toilet rooms, dressing rooms, refuse rooms, mechanical rooms, under sewer lines that are not shielded to intercept potential drips, under leaking water lines, including leaking automatic fire sprinklers heads, or under lines on which water has condensated, under open stairwells or under sources of contamination. Conditions Observed: Multiple surfaces around bulk coke cylinders had syrup residue sprayed onto them. Wash all surfaces impacted by this spill/spray. Soft serve nozzle head had heavy spillage of product. Wash, rinse sanitize these surface frequently enough to maintain sanitary.

42. Garbage and refuse properly disposed; facilities maintained.

Conditions Observed: Dumpsters were overfilled and debris had spilled onto ground. Contact your waste hauler when dumpsters are approaching full; lids should always be able to close fully. Debris was observed on ground around dumpsters. Pick up debris and keep this area clean of spillage and soiling.

Conditions Observed: Measured butter 47F and 47.5F with probe thermometer in BevAir under counter refrigerator. Adjust/repair/replace BevAir under counter refrigerator to hold cold potentially hazardous foods at or below 41F at all times.

ROACHY SAYS:

“Except during preparation, cooking, cooling, transportation to or from a retail food facility for a period of less than thirty minutes, when time is used as the public health control, or as otherwise specified, potentially hazardous foods shall be maintained at or below 41F or at or above 135F. The following foods may be held at or below 45F: raw shell eggs, unshucked live molluscan shellfish, pasteurized milk and pasteurized milk products in original sealed containers, potentially hazardous foods held during transportation. Potentially hazardous foods held for dispensing in serving lines and salad bars may be held at 45F, not to exceed 12 hours in a 24 hour period, if unused portions are discarded. Conditions Observed: Observed mold-like substance in ice machine. Clean and sanitize ice machine per manufacturer’s instructions and maintain this piece of equipment to prevent mold growth. Observed mold-like substance in soda nozzles at soda machine (on the right) in the dining area. Remove and soak soda nozzles every day to prevent mold growth.

ROACHY SAYS:

“Food-contact surfaces and utensils shall be clean to sight and touch. Food-contact surfaces and multiservice utensils shall be effectively washed, rinsed, and sanitized to remove or completely loosen soils by use of manual or mechanical methods. Precleaning may be required. Manual sanitization shall be accomplished during the final rinse by contact with solution containing 100 ppm available chlorine, 200 ppm quaternary ammonium, or another approved sanitizer. Mechanical sanitization shall be accomplished in the final rinse by achieving a utensil surface temperature of 160F or by contact with solution containing 50 ppm available chlorine, 200 ppm quaternary ammonium, or another approved sanitizer. After cleaning and sanitizing, equipment and utensils shall be air dried. Mechancial warewash machines must be approved and installed and operated according to manufacturer's specifications. Equipment, food contact surfaces, and utensils must be cleaned and sanitized throughout the day at least every four hours or as needed to prevent contamination. Conditions Observed: Observed chemical spray bottle hanging on wire rack adjacent to lids and sauces. Store chemicals away from food, food-contact surfaces, utensils, or packaging materials to protect these items from contamination. Manager moved chemical spray bottle to another location. Thank you for correcting this minor violation.

Conditions Observed: Observed food debris on blender machine, espresso machine, interior of BevAir under counter refrigerator, ice bin exterior at drive-thru, grease on wheels of charbroiler, syrup in syrup compartment of soft-serve machine, shelf under microwave adjacent to Traulsen freezer, grease on hood baffle filters over all cooking equipment, floor sink adjacent to water heater, grease on top of grease container. Clean non-food contact surfaces regularly to prevent buildup of grease and/or food debris.

Multi-use food contact surfaces must be smooth, free of breaks, chips, and similar imperfections; free of sharp internal angles, corners, and crevices; finished to have smooth welds and joints; be accessible for cleaning and inspection. Conditions Observed: Observed tongs stored on top of soiled French fry hot hold. Observed ice scoop stored in ice inside of ice machine. Observed ice scoop at drive-thru stored in soiled container. Store utensils in sanitary containers and maintain clean. the ice scoop was moved to the ice scoop holder inside of the ice machine and the soiled ice scoop at the drive-thru was sent to warewashing. The walls and ceiling shall be smooth, durable, nonabsorbent, and easily cleanable. Conditions Observed: Observed several items held out of temperature control lacking time labels (French fry cheese, sliced tomatoes, sliced cheese). Observed sunday toppings with use by stickers 11/28/18 still in use. When using time as a public health control, you must time label all foods held out of temperature control and then discard them after the time has past. This is a major violation that will require a reinspection on or after 12/7/18.

ROACHY SAYS:

“When time as a public health control is used, the following procedures shall be observed: food items shall be marked to indicate four hours past the time when the food was removed from temperature control, food shall be served or discarded within the four hour time limit. Written procedures shall be maintained in the food facility and made available to the enforcement agency upon request, that ensure compliance with this section and section 114002, for food that is prepared, cooked, and refrigerated before time is used as a public health control. 41. Plumbing: proper backflow devices.

Conditions Observed: Observed non-functional handwash sink (rear cookline). Repair/replace handwash sink to provide adequate facilities for handwashing. Send service invoice via email or fax when repaired.

ROACHY SAYS:

“Steam tables, ice machines, ice bins, food preparation sinks, display cases, walk-in refrigeration units, and other similar equipment that discharge liquid waste shall drain into a floor sink with an air gap or other approved type of receptor. All plumbing and plumbing fixtures shall be installed in compliance with applicable local plumbing ordinances, maintained so as to prevent any contamination, kept clean, fully operative, and in good repair. The potable water supply shall be protected with a backflow or back siphonage protection device. Any hose used for potable water shall be of approved materials, labeled, properly stored, and used for no other purpose. 42. Garbage and refuse properly disposed; facilities maintained.

Conditions Observed: Observed garbage and debris surrounding dumpster area. Keep dumpster area clean and free of debris to minimize vermin attraction, public health hazard or nuisance.

ROACHY SAYS:

“The exterior premises of a food facility shall be kept free of litter and rubbish. All food waste and rubbish shall be kept in nonabsorbent, durable, cleanable, leak-proof, and rodent-proof containers. Containers shall have close-fitting lids and be covered at all times. All waste must be removed and disposed of as frequently as necessary to prevent the development of objectionable odors and other conditions that attract or harbor insects or rodents. 45. Floor, walls, and ceilings: built, maintained, clean.

Conditions Observed: Observed grease accumulation and/or food debris on floors under equipment. Clean and sanitize floors regularly to prevent buildup.

ROACHY SAYS:

“Food facilities shall be fully enclosed in a building consisting of permanent floors, walls, and an overhead structure that meets minimum standards. The walls and ceiling shall be smooth, durable, nonabsorbent, and easily cleanable. Flooring and base coving shall be smooth, durable, and made of approved nonabsorbent material that is easily cleanable. Floor surfaces shall be coved at the juncture of the floor and wall with a 3/8 inch minimum radius and shall extend up the wall at least 4 inches.
