Conditions Observed: Unable to verify current FSM certificate during the inspection (posted FSM certificate is expired). Obtain FSM certificate within 60 days and make available at the facility for review during routine inspections.

“Food facilities that prepare, handle, or serve non-prepackaged potentially hazardous food, shall have an owner or employee who has passed an aproved food safety certification examination. 08. Proper hot and cold holding temperatures.
Conditions Observed: Measured butter 65F, stored on a shelf held out of temperature control, lacking a time label. You may hold butter out of temperature control if it is time labeled for 4 hours and then discarded after that 4 hours has past. Or, maintain cold PHF at/below 41F at all times. Per operator, the butter was out of temperature for less than 2 hours; operator returned butter to the refrigerator, this major violation was corrected. Measured 1 container of chicken cordon bleu 53F and another container of chicken cordon bleu 59F, in customer self-service cold bar. All other cold PHF in this unit measured at/below 41F. Revise cold holding procedures to maintain cold PHF at/below 41F at all times. Operator discarded 2 containers of chicken cordon bleu, this major violation was corrected. Measured 2 containers of BBQ meatballs 48F in customer self-service cold bar. Measured pot pie and asiago pasta salad 44F in deli cold case. Adjust/service/repair cold bar and cold case to hold cold PHF at/below 41F at all times.

Conditions Observed: Observed 2 containers of baby food offered for sale after expiration date had past. Ensure proper rotation of foods when stocking shelves with new product and check expiration dates regularly. 2 expired baby food products were removed from the sales floor.

Conditions Observed: Deli has installed a flat grill that does not fit under the existing ventilation hood or ansul system. Per meat dept. manager, the facility is planning to install a new ventilation hood that will be large enough to include the flat grill. Be advised that the addition of a new ventilation hood requires approval from this office. Submit a 1-hour plan check to this office for the propose ventilation hood for approval prior to purchase and installation.

“Approved exhaust hoods shall be provided to remove toxic gases, heat, grease, vapors and smoke. Canopy-type hoods shall extend six inches beyond all cooking equipment. Toilet rooms shall be consistent with local building codes and vented to the outside air by a screened openable window, air shaft, or light-switch activated exhaust fan. Adequate lighting shall be provided in all areas to facilitate cleaning and inspection. Light fixtures in areas where open food is stored, served, prepared, and where utensils are washed shall be of shatterproof construction or protected with light shields. Conditions Observed: Measured macaroni and cheese 118F holding in deli hot case. Per deli manager, macaroni and cheese was prepared and holding in the hot case for less than 2 hours. Instructed deli manager to reheat macaroni and cheese to at least 165F, then monitor and log holding temperature of foods held in the hot case throughout the day. Facility has plans to replace this piece of equipment soon. This major violation was corrected. Measured sliced strawberries 44F, holding in deli cold bar. Do not stack containers of potentially hazardous foods (phf) in the deli cold bar; maintain phf in direct contact with refrigeration to keep foods at/below 41F at all times.

Conditions Observed: Observed deli employee use gloved hands to put raw ground beef in sauté pan on the stove then change contaminated gloves without handwashing, as required. Handwashing must occur after handling raw potentially hazardous foods from animal origin and prior to donning gloves. Instructed employee to handwash. Deli employees demonstrated active handwashing and proper glove use through the remainder of the inspection. Deli manager to hold refresher training on handwashing and glove use with all deli employees. This major violation was corrected on site, thank you.

Conditions Observed: Observed large trash can obstructing handwash sink in deli. The trash can is on wheels and is easy to move, however hands are contaminated when moving trash can. Consider moving the trash can to another location to keep handwash sink unobstructed for proper handwashing at all times.

Conditions Observed: Measured several rotisserie meat items holding below 120F in rotisserie hot hold unit. Measured rotisserie chicken 104F, rotisserie chicken 96.9F, chicken breast 117F and pork ribs 88F with probe thermometer in hot hold unit. Observed product stacked in hot hold unit. Measured ambient air 198.5F in same location. Consider stocking less food in this unit to ensure it is holding at or above 135F at all times. Employee removed all food items from rotisserie hot hold and put them into walk-in freezer for rapid cooling. Employees will continue to monitor hot hold unit and will cook new product in the rotisserie oven for hot holding. Thank you for correcting this major violation.

Conditions Observed: Observed non-functioning thermometer in rotisserie hot hold unit. Replace the thermometer to visibly monitor hot holding temperature to verify it is at or above 135F at all times.
ROACHY SAYS:
“An accurate easily readable metal probe thermometer suitable for measuring temperature of food shall be available to the food handler. Thermometers shall be calibrated in accordance with manufacturer's specifications as necessary to ensure their accuracy. A thermometer +/- 2 degrees F shall be provided for all refrigerators, hot and cold holding units, and high temperature warewashing machines. (114157, 114159)”
41. Plumbing: proper backflow devices.
Conditions Observed: Observed broken pipe leaking water on to the floor adjacent to water heater and ice machine. Repair leak.
ROACHY SAYS:
“Steam tables, ice machines, ice bins, food preparation sinks, display cases, walk-in refrigeration units, and other similar equipment that discharge liquid waste shall drain into a floor sink with an air gap or other approved type of receptor. All plumbing and plumbing fixtures shall be installed in compliance with applicable local plumbing ordinances, maintained so as to prevent any contamination, kept clean, fully operative, and in good repair. The potable water supply shall be protected with a backflow or back siphonage protection device. Any hose used for potable water shall be of approved materials, labeled, properly stored, and used for no other purpose.
