Conditions Observed: No paper towels in the dispenser at the front handwashing sink. Employee refilled dispenser - COS. Keep handwashing sinks stocked with paper towels and pump soaps at all times.

Handwashing facilities shall be clean, unobstructed, and accessible at all times. Handwashing facilities shall be provided with soap and sanitary single use towels in approved dispensers. 08. Proper hot and cold holding temperatures.

Conditions Observed: Measured noodles in the steamer to be 118 F. Revise procedure to hot hold noodles at 135 F or above. Measured peanut sauce in the hot hold to be 118 F. Throughly heat peanut sauce to 165 F before placing in hot hold. Hot hold at or above 135 F. Observed a container of mussels a shelf with no temperature control. Per employee, mussels were out for today’s lunch service. Keep mussels in a fridge at or below 41 F. This violation requires a reinspection on/after 12/3/21.

Conditions Observed: Observed utensils for rice resting in containers of water. Rest utensil on a clean and dry food contact surface or in water or above 135 F.

During pauses in food preparation utensils shall be stored with handles above the top of the food, on a clean sanitized surface, in running water, or in water that is at least 135øF. Conditions Observed: Observe water pooling in the recessed area where a tile was damaged (in the floor near the top of the cookline). Repair or replace tile to eliminate standing water.

Conditions Observed: Observed cook handle their cell phone and touch their face after washing their hands and prior to resuming food service. Inspector intervened and directed employee to wash their hands - COS. Train all employees to avoid handling cell phones and similar phomites during food service. Review handwashing requirements with all kitchen staff (i.e., after touching one’s face).

Conditions Observed: Observed cook add raw beef to bowl of pho. Observed cook use same tongs to add cilantro to pho and then use same tongs to add more raw beef to bowl before returning tongs to cilantro. Observed cilantro as a ready-to-eat food in a to-go order next to the pho. Inspector intervened; observed remaining cilantro was discarded and requested that insert and tongs be brought to the 3-part sink. Designate a pair of tongs for use with raw beef only. Use a separate pair of tongs to for the cilantro. Train kitchen employees in how to prevent cross-contamination of ready-to-eat foods. This inspection requires a reinspection on/after 7/23/21.

ROACHY SAYS:

“Food shall be produced, prepared, stored, transported, and served so as to be free from adulteration and spoilage. Employees shall not commit any act that may cause the contamination or adulteration of food or food contact surfaces. Containers previously used to store poisonous or toxic materials shall not be used to store food, utensils, or single use articles. Utensils used for tasting shall be discarded or washed, rinsed, and sanitized between tastings. Ice used as a medium for cooling food and beverages shall not be offered for consumption. Food shall be inspected upon receipt and prior to any use, storage, or resale. Shell eggs shall be received clean, sound, and not exceed restricted tolerances for US Consumer Grade B Standards. Conditions Observed: Measured 0 PPM in the dishwasher today. Adjust/service/repair to deliver 50 PPM in each cycle. Wash, rinse, and sanitize dishes via the 3-compartment method until dishwasher is operational. This violation requires a reinspection on/after 7/23/21.

Food-contact surfaces and multiservice utensils shall be effectively washed, rinsed, and sanitized to remove or completely loosen soils by use of manual or mechanical methods. Precleaning may be required. Manual sanitization shall be accomplished during the final rinse by contact with solution containing 100 ppm available chlorine, 200 ppm quaternary ammonium, or another approved sanitizer. Mechanical sanitization shall be accomplished in the final rinse by achieving a utensil surface temperature of 160F or by contact with solution containing 50 ppm available chlorine, 200 ppm quaternary ammonium, or another approved sanitizer. Conditions Observed: Observed door on front of 2-door fridge on the cookline to be in disrepair. Repair door to close tightly.

Conditions Observed: Measured large vat of pho in the walkin to be 43 F (center), 44 F F (edge). Per Owner, pho was prepared yesterday. Revise cooling method to so pho reaches 41 F or below within in 6 hours. See regulation description. This violation requires a reinspection on/after 3/10/2021.

ROACHY SAYS:

“After heating, potentially hazardous foods shall be rapidly cooled from 135F to 70F within 2 hours, and then from 70F to 41F within 4 hours. Potentially hazardous foods prepared from ambient temperature ingredients must be cooled to below 41F within 4 hours. Cooling shall be facilitated by one or more of the following methods: in shallow pans, separating food into smaller portions, adding ice as an ingredient, using an ice bath and stirring frequently, using an ice paddle, using rapid cooling equipment, or using containers that facilitate heat transfer. 22. Hot and Cold Water Available

Conditions Observed: No warm water available at the front handwashing sink today; faucet was turned off due to a leak. Repair sink to deliver both warm (100 to 108 F) and cool water.

Conditions Observed: Observed build-up on interior, edges of tops of bulk ingredient containers. Showed observations to Owner. Maintain tidy containers. Observed build-up, staining on plastic trash bin used to store bulk rice. Discard container. Store rice in a clean, food grade container. Observed rice being reheated in a plastic shopping bag. Employee decanted rice into a clean bowl - COS. Use food grade containers and clean utensils to reheat food. Plastic shopping bags are not okay.

Conditions Observed: Measured foods in the walkin to be above 41 F (shell egg, 46 F; beef, 44 F). Thermometer reads 45 F. Adjust/service/repair to cold-hold foods at 41 F or below.

“Except during preparation, cooking, cooling, transportation to or from a retail food facility for a period of less than thirty minutes, when time is used as the public health control, or as otherwise specified, potentially hazardous foods shall be maintained at or below 41F or at or above 135F. The following foods may be held at or below 45F: raw shell eggs, unshucked live molluscan shellfish, pasteurized milk and pasteurized milk products in original sealed containers, potentially hazardous foods held during transportation. Potentially hazardous foods held for dispensing in serving lines and salad bars may be held at 45F, not to exceed 12 hours in a 24 hour period, if unused portions are discarded. 10. Proper cooling methods.

Conditions Observed: Measured beef cooked yesterday to be 47 F, 48 F. Employee discarded - COS. Revise cooling procedure to rapidly cool large pieces of meat (e.g. cut into smaller pieces, allow meat to rest uncovered while cooling). Avoid stacking warm foods. This violation requires a reinspection on/after 12/4/19.

ROACHY SAYS:

“After heating, potentially hazardous foods shall be rapidly cooled from 135F to 70F within 2 hours, and then from 70F to 41F within 4 hours. Potentially hazardous foods prepared from ambient temperature ingredients must be cooled to below 41F within 4 hours. Cooling shall be facilitated by one or more of the following methods: in shallow pans, separating food into smaller portions, adding ice as an ingredient, using an ice bath and stirring frequently, using an ice paddle, using rapid cooling equipment, or using containers that facilitate heat transfer. 15. Food contact surfaces: clean and sanitized.

Conditions Observed: Observed food debris on plates stacked by rice steamer. Observed debris on blender lids. These items sent to warewash - COS. One chipped bowl was discarded. Maintain all utensils to be clean to the sight and clean to the touch.

Food-contact surfaces and multiservice utensils shall be effectively washed, rinsed, and sanitized to remove or completely loosen soils by use of manual or mechanical methods. 25. Person in charge present and performs duties.

Conditions Observed: No Person-In-Charge (PIC) when Inspector first arrived. Designate and train a PIC for each shift.

ROACHY SAYS:

“A Person in Charge (PIC) shall be present at the food facility during all hours of operation. The PIC shall ensure that persons unecessary to the food facility operation shall not be allowed in food preparation, food storage, or warewashing areas. The PIC shall ensure that consumers are notified that clean tableware is to be used when they return to self-service areas such as salad bars and buffets. (113945, 113945.1, 113984.1, 114075(c))”

28. Food separated and protected.

Conditions Observed: Observed build-up on interior, edges of tops of bulk ingredient containers. Showed observations to Owner. Maintain tidy containers. Observed build-up, staining on plastic trash bin used to store bulk rice. Discard container. Store rice in a clean, food grade container. Observed rice being reheated in a plastic shopping bag. Employee decanted rice into a clean bowl - COS. Use food grade containers and clean utensils to reheat food. Plastic shopping bags are not okay.

“Food shall be protected from contamination during transportation, storage, preparation, holding, and display. Adequate and suitable counter space shall be provided for all food preparation. Conditions Observed: That’s pretty lame about your pest control provider.

Conditions Observed: Observed floor below 3-part sink to be excessively damp; observed build-up below 3part sink, dishwasher. As discussed during the inspection, maintain a clean and dry surfaces.

Conditions Observed: Observed employee wearing gloves add veggies to a bowl, open the fridge door with gloved hands, add dressing to the veggies, and then toss salad with the same gloves. Change gloves after touching surfaces like refrigerator handles. Use a utensil to toss salad.

Conditions Observed: Unable to locate a thermometer in all refrigeration units. Provide a thermometer for all units.

Thermometers shall be calibrated in accordance with manufacturer's specifications as necessary to ensure their accuracy. 41. Plumbing: proper backflow devices.

Conditions Observed: Handwashing sinks at front station were not operational today. Per staff, they were turned off due to a leak. Repair leak so handwashing sinks are operational.

“Steam tables, ice machines, ice bins, food preparation sinks, display cases, walk-in refrigeration units, and other similar equipment that discharge liquid waste shall drain into a floor sink with an air gap or other approved type of receptor. All plumbing and plumbing fixtures shall be installed in compliance with applicable local plumbing ordinances, maintained so as to prevent any contamination, kept clean, fully operative, and in good repair. The potable water supply shall be protected with a backflow or back siphonage protection device. Any hose used for potable water shall be of approved materials, labeled, properly stored, and used for no other purpose. Conditions Observed: Observed employee on the cookline cough continuously and use hands to catch cough. See #5 below. Employees who can not control coughs or discharge from the eyes, nose, and mouth may not work with food or clean utensils. Develop a written employee wellness policy and retrain all in employees in employee health. This violation requires a reinspection on/after 3/1/2019.

Conditions Observed: Observed employee cover a cough with their bare hands and then add veggies to an order. No handwashing observed. Always wash hands after cough. Observed an employee portioning raw chicken, put chicken in walkin and then return to the cookline. Inspector instructed employee to wash their hands; observed okay handwashing. Train all employees in handwashing as required by CA Retail Food Code.

09. Time as public health control; procedures and records.

Conditions Observed: Observed several foods at ambient temperature lacking time labeling (egg rolls above fryer, spring rolls, shell eggs, strawberries). Foods shall be marked with the time at which four hours have passed since the food was removed from temperature control. A written policy for all Time as Public Health Control foods shall be maintained at the facility and made available to the Inspector. Consider keeping foods at or below 41 F or cooking to order.

ROACHY SAYS:

“When time as a public health control is used, the following procedures shall be observed: food items shall be marked to indicate four hours past the time when the food was removed from temperature control, food shall be served or discarded within the four hour time limit. Written procedures shall be maintained in the food facility and made available to the enforcement agency upon request, that ensure compliance with this section and section 114002, for food that is prepared, cooked, and refrigerated before time is used as a public health control. Conditions Observed: Measured water at the 3part sink to be 113.5 F. Adjust/service/repair to deliver hot water at 120 F or greater.

“An adequate, protected, pressurized, approved potable supply of hot and cold water shall be provided at all times. Hot water shall be supplied at a minimum temperature of 120F when measured from the faucet. The temperature of water provided to handwashing facilities shall be at least 100F. Handwashing facilities equipped with non-adjustable faucets shall have their temperature set between 100F and 108F. Hot water shall be provided at the food preparation sink and janitorial sink. 34. Non-food-contact surfaces clean.

Conditions Observed: Observed excessive build-up and debris throughout the facility: behind food inserts in the deli prep, below shelving the in the food storage area, at the junction of wall/floor in the food storage area, on the the outside of bulk storage containers. Remove build-up; clean and sanitize impacted surfaces.

ROACHY SAYS:

“Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris. (114115(b))”
