06. Hands clean and properly washed; gloves used properly.

Conditions Observed: Observed cookline employee fail to wash their hands after using a gloved hand to put ground beef on the grill. Hands must be washed after handling raw animal proteins, after changing tasks, prior to donning a new pair of gloves prior to conducting food prep, and anytime in which the hands may become contaminated. Instructed employee to wash their hands. Handwashing was demonstrated by cookline employees over the remainder of the inspection.

ROACHY SAYS:

“Food employees shall minimize bare hand and arm contact with nonprepackaged food that is in ready to eat form. Whenever gloves are worn, they shall be changed, replaced, or washed as often as handwashing is required. Single-use gloves shall not be washed. Employees shall wash their hands in the following situations: immediately before engaging in food preparation including working with nonprepackaged food, clean equipment and utensils and unwrapped single-use food containers and utensils; after touching bare human body parts other than clean hands and clean exposed portions of arms; after using the toilet room; after caring for or handling any animal; after coughing, sneezing, using a handkerchief or disposable tissue, using tobacco, eating or drinking; after handling soiled equipment or utensils; during food preparation, as often as necessary to remove soil and contamination and to prevent cross-contamination when changing tasks; when switching from working with raw food and working with ready-to-eat foods; before initially donning gloves for working with food; before dispensing or serving food handling clean tableware and serving utensils in the food service area; after engaging in other activities that contaminate the hands. 08. Proper hot and cold holding temperatures.

Conditions Observed: Measured sliced tomatoes 47F, rice and crushed garlic 46F in 2 door prep table. Measured orzo 48F, rice 47F and curry 46F in walk-in refrigerator. Measured ambient air of walk-in refrigerator 43.7F. Per manager, the walk-in was holding above required temperature and was serviced approximately 2 hours prior to the inspection. Continue to monitor walk-in; adjust/service/repair both units to hold cold foods at or below 41F at all times.

15. Food contact surfaces: clean and sanitized.

Conditions Observed: Measured QT sanitizer 0 PPM at sanitizer container at the espresso bar. Maintain QT sanitizer between 200-400PPM to effectively sanitize food contact and non-food contact surfaces. Manager refreshed QT sanitizer solution that measured 200PPM.

ROACHY SAYS:

“Food-contact surfaces and utensils shall be clean to sight and touch. Food-contact surfaces and multiservice utensils shall be effectively washed, rinsed, and sanitized to remove or completely loosen soils by use of manual or mechanical methods. Precleaning may be required. Manual sanitization shall be accomplished during the final rinse by contact with solution containing 100 ppm available chlorine, 200 ppm quaternary ammonium, or another approved sanitizer. Mechanical sanitization shall be accomplished in the final rinse by achieving a utensil surface temperature of 160F or by contact with solution containing 50 ppm available chlorine, 200 ppm quaternary ammonium, or another approved sanitizer. After cleaning and sanitizing, equipment and utensils shall be air dried. Mechancial warewash machines must be approved and installed and operated according to manufacturer's specifications. Food contact surfaces, utensils, and equipment shall be cleaned and sanitized at the following times: before each use with different type of raw food of animal origin, when changing from raw food to ready to eat food, between uses with raw produce and potentially hazardous food, before using a thermometer, and any time during the operation when contamination may have occurred. Warewash sinks used to wash wiping cloths, wash produce, or thaw food must be cleaned and sanitized before and after these uses. Equipment, food contact surfaces, and utensils must be cleaned and sanitized throughout the day at least every four hours or as needed to prevent contamination. 20. Consumer advisory provided for raw or undercooked foods.

Conditions Observed: Facility offers burgers grilled to order and a seared ahi tuna melt and does not have a consumer advisory warning customers of the increase risk with consuming raw and/or undercooked foods. Provide a consumer advisory on the menu warning customers that, “consuming raw or undercooked meats, poultry, seafood, shellfish or eggs may increase your risk to foodborne illness, especially if you have certain medical conditions.”

ROACHY SAYS:

“If a food of animal origin is served raw, undercooked, or without otherwise being processed to eliminate pathogens, either in ready-to-eat form or as an ingredient in another ready-to-eat food, the permitholder shall inform consumers of the significantly increased risk of consuming those foods by way of a disclosure pursuant to subdivision (b) of 114093 and reminder pursuant to subdivision (c) of 114093, using brochures, deli case or menu advisories, label statements, table tents, placards, or other effective written means. Conditions Observed: Measured herb garlic butter 82F, held out of temperature control on the cookline. Garlic and oil mixtures, including butter are a potentially hazardous food that must be held at/below 41F at all times. This food was discarded and will now be held under temperature control in the prep table. Queso fresca measured 60F, held in a double stacked plastic container insert in top portion of cookline prep table. Do not stack insert containers over uncovered food. Food in top insert container is not in direct contact with refrigeration and is not maintained cold. Cheese was discarded and insert containers will no longer be stacked in the prep table.

Conditions Observed: Measured cubed ham 44.5F and blue cheese 46.5F with probe thermometer in walk-in refrigerator. Adjust/service/repair walk-in to hold cold potentially hazardous foods at or below 41F at all times. Measured pest from one container 47.5F and pesto from another container 48F with probe thermometer in Traulsen 2 door catering refrigerator. This refrigerator has been marked out of compliance on the routine inspections conducted on 11/14/17, 3/27/18, 8/28/18, 1/8/19, 7/17/19 and today. Please consider retiring this unit and replacing with a commercial unit capable of holding temperatures at or below 41F at all times.

10. Proper cooling methods.

Conditions Observed: Measured clam chowder 93.5F with probe thermometer cooling in walk-in refrigerator in the cafe. The clam chowder was prepared at the baker today and the cooling process began at approximately 2PM. An hour had past when the clam chowder was measured at 93.5F. Observed clam chowder stored in a covered plastic container. You must rapidly cool potentially hazardous foods from 135F - 70F in 2 hours and from 70F - 41F within 4 hours (6 total). Instructed Catering Manager to remove the soup from the walk-in and complete the cooling process. Observed soup had been split into 2 metal bowls, placed in an ice bath with cooling wands. Both bowls of clam chowder measured below 41F with probe thermometer at 4:15PM. Please use the cooling log to ensure you are rapidly cooling potentially hazardous foods within the required time frame(s). This major violation was corrected on site, thank you.

ROACHY SAYS:

“After heating, potentially hazardous foods shall be rapidly cooled from 135F to 70F within 2 hours, and then from 70F to 41F within 4 hours. Potentially hazardous foods prepared from ambient temperature ingredients must be cooled to below 41F within 4 hours. Cooling shall be facilitated by one or more of the following methods: in shallow pans, separating food into smaller portions, adding ice as an ingredient, using an ice bath and stirring frequently, using an ice paddle, using rapid cooling equipment, or using containers that facilitate heat transfer. 22. Hot and Cold Water Available

Conditions Observed: Hot water measured 104.5F with probe thermometer at 3 compartment sink at the beginning of the inspection. Water heater temperature control knob was moved by a trash can stored in this location, which turned the water temperature down. Recommend covering the water heater control knob, or moving the trash can, to ensure hot water is provided at or above 120F at all mixing faucets at all times. Water heater temperature was adjusted and hot water measured 117.5F at the end of the inspection.

“An adequate, protected, pressurized, approved potable supply of hot and cold water shall be provided at all times. Hot water shall be supplied at a minimum temperature of 120F when measured from the faucet. The temperature of water provided to handwashing facilities shall be at least 100F. Handwashing facilities equipped with non-adjustable faucets shall have their temperature set between 100F and 108F. Hot water shall be provided at the food preparation sink and janitorial sink. Conditions Observed: Observed fly strip hung over food prep area in front of the bakery and fly strip hung over bulk flour bags in bakery. Remove fly strips from locations where food and utensils are stored and/or handled.

This refrigerator has been marked out of compliance on the routine inspections conducted on 11/14/17, 3/27/18, 8/28/18, 1/8/19 and today. Recommend replacing this refrigerator with another commercial unit capable of holding cold potentially hazardous foods at or below 41F at all times. Submit spec sheets for proposed refrigerator to this office for approval prior to purchase and installation.

Conditions Observed: Observed approximately 15-25 flies in bakery facility. Increase pest control efforts to remove flies from the facility; Recommend contacting professional pest control service. This is a major violation that will require a reinspection on or after 7/22/19.

Conditions Observed: Observed uncovered food stored in walk-in refrigerator and walk-in freezer. Cover all stored foods to prevent contamination and/or freezer burn. Observed open bulk bags of food stored on the shelf. Once a bulk bag of food is opened, transfer it to a rigid container with a lid to prevent contamination and/or vermin attraction.

ROACHY SAYS:

“Food shall be protected from contamination during transportation, storage, preparation, holding, and display. Adequate and suitable counter space shall be provided for all food preparation. Food preparation shall be conducted wthin an approved food compartment or fully enclosed food facility. Open-air barbecues shall be separated from public access. 31. Food storage; food storage containers identified.

Conditions Observed: Observed food stored on the floor in walk-in freezer. Store food at least 6 inches above the floor to prevent contamination and potential vermin attraction.

ROACHY SAYS:

“Adequate and suitable space shall be provided for the storage of food. Food shall be protected from contamination by storing the food in a clean, dry location, where it is not exposed to splash, dust, vermin, or other forms of contamination or adulteration, and at least six inches above the floor. Food shall not be stored in any of the following ways: in locker rooms, toilet rooms, dressing rooms, refuse rooms, mechanical rooms, under sewer lines that are not shielded to intercept potential drips, under leaking water lines, including leaking automatic fire sprinklers heads, or under lines on which water has condensated, under open stairwells or under sources of contamination. Conditions Observed: Observed moldy strawberries and moldy cheese stored in walk-in refrigerator. Label and date all stored foods for proper rotation. Please discard moldy foods.

ROACHY SAYS:

“Any food is misbranded if its labeling is false or misleading, if it is offered for sale under the name of another food, or if it is an imitation of another food for which a definition and standard of identity has been established by regulation. A retail food facility shall not sell or offer for sale infant formula or baby food after the "use by" date. Food prepackaged in a food facility shall bear a label that complies with the labeling requirements prescribed by the Sherman Food, Drug, and Cosmetic Law. A franchise food chain with 20 or more stores must disclose calories, carbohydrates, saturated fat, and sodium for each standard menu item. Conditions Observed: Observed unapproved utensils (plastic cups, bowls, etc.) used to portion bulk food items. Obtain approved utensils with handles that are smooth, durable, nonabsorbent and easy to clean. Observed failed rubber gasket on walk-in refrigerator door. Repair. Observed leaking oil on standing mixer that may contaminate food. Do not use this piece of equipment until it is repaired. Observed out of order sign posted on mixer.

ROACHY SAYS:

“Utensils and equipment shall be kept fully operative and in good repair. All new and replacement equipment shall be properly installed and be certified or classified for sanitation by an ANSI accredited certification program. Electrical appliances must meet UL standards. Single-use articles shall not allow migration of deleterious substances or impart colors, odors, or tastes to food. Multi-use food contact surfaces must be smooth, free of breaks, chips, and similar imperfections; free of sharp internal angles, corners, and crevices; finished to have smooth welds and joints; be accessible for cleaning and inspection. Unless specified, wood and wood wicker may not be used as a food-contact surface. Unless specified, copper and copper alloys such as brass may not be used in contact with a food that has a pH below six or for a fitting between a backflow preventer and a carbonator. Conditions Observed: Observed unapproved utensils fully submerged in bulk foods or stored on top of soiled bulk food containers. Obtain utensils with handles, and store with handles facing up out of contact with food to prevent contamination.

“Clean equipment, utensils, linens, and single-use articles shall be stored at least six inches above the floor in an approved location within the fully enclosed permitted food facility. Clean equipment, utensils, linens, and single-use articles shall not be exposed to splash, dust, vermin, or other forms of contamination. Non-food items shall be stored and displayed separate from food and food-contact surfaces. During pauses in food preparation utensils shall be stored with handles above the top of the food, on a clean sanitized surface, in running water, or in water that is at least 135øF. Keep doors and windows closed. Seal all ingress holes and install door skirts at the bottom of doors with gaps. Obtain insect electrocution devices that retain the insects and install away from food and utensil storage and handling areas. Keep surfaces clean, dry and free of food debris/sugar to minimize flies.

28. Food separated and protected.

Conditions Observed: Observed several open bulk bags of food lacking containers. Once a bulk bag of food is open, transfer it to a rigid container with a tight-fitting lid to protect bulk foods from contamination.

ROACHY SAYS:

“Food shall be protected from contamination during transportation, storage, preparation, holding, and display. Adequate and suitable counter space shall be provided for all food preparation. Food preparation shall be conducted wthin an approved food compartment or fully enclosed food facility. Open-air barbecues shall be separated from public access. 31. Food storage; food storage containers identified.

Conditions Observed: Observed food on the floor in walk-in freezer. Store foods at least 6 inches above the floor to protect foods from contamination.

ROACHY SAYS:

“Adequate and suitable space shall be provided for the storage of food. Food shall be protected from contamination by storing the food in a clean, dry location, where it is not exposed to splash, dust, vermin, or other forms of contamination or adulteration, and at least six inches above the floor. Food shall not be stored in any of the following ways: in locker rooms, toilet rooms, dressing rooms, refuse rooms, mechanical rooms, under sewer lines that are not shielded to intercept potential drips, under leaking water lines, including leaking automatic fire sprinklers heads, or under lines on which water has condensated, under open stairwells or under sources of contamination. Working containers holding food or food ingredients that are removed from their original packages for use in the food facility shall be identified with the common name of the food. Non-prepackaged food may not be stored in direct contact with undrained ice. Products that are held by the permit holder for credit, redemption, or return to the distributor, such as damaged, spoiled, or recalled products shall be segregated and held in designated areas that are separated from food, equipment, utensils, linens, and single-use articles. 36. Equipment / Utensils approved; installed, clean, good repair, capacity.

Conditions Observed: Observed failed rubber gasket on walk-in refrigerator door. Repair rubber gasket. Observed peeling paint on standing mixer and a sign warning employees that this piece of equipment is leaking oil that could potentially contaminate food. Immediately stop using this piece of equipment until it is repaired to protect foods from contamination.

ROACHY SAYS:

“Utensils and equipment shall be kept fully operative and in good repair. All new and replacement equipment shall be properly installed and be certified or classified for sanitation by an ANSI accredited certification program. Electrical appliances must meet UL standards. Single-use articles shall not allow migration of deleterious substances or impart colors, odors, or tastes to food. Multi-use food contact surfaces must be smooth, free of breaks, chips, and similar imperfections; free of sharp internal angles, corners, and crevices; finished to have smooth welds and joints; be accessible for cleaning and inspection. Unless specified, wood and wood wicker may not be used as a food-contact surface. Unless specified, copper and copper alloys such as brass may not be used in contact with a food that has a pH below six or for a fitting between a backflow preventer and a carbonator. Beverage tubing and cold-plate beverage cooling devices cannot be installed in contact with stored ice intended to be used as food. Equipment that is not movable must be installed to allow for cleaning in place. Cutting or piercing parts of can openers must be readily removable. Facilities that wash, rinse, soak, thaw, or similarly prepare foods shall be provided with a approved food preparation sink. Reservoirs that supply water to produce foggers must be cleaned and maintained. Surfaces such as cutting blocks and boards that are subject to scratching and scoring shall be resurfaced or replaced if they can no longer be effectively cleaned and sanitized. 37. Equipment, utensils and linens: storage and use.

Conditions Observed: Observed rolling racks stored outside back of facility in the rain. Clean and sanitize racks, and store in a protected environment.

ROACHY SAYS:

“Clean equipment, utensils, linens, and single-use articles shall be stored at least six inches above the floor in an approved location within the fully enclosed permitted food facility. Clean equipment, utensils, linens, and single-use articles shall not be exposed to splash, dust, vermin, or other forms of contamination. Non-food items shall be stored and displayed separate from food and food-contact surfaces. During pauses in food preparation utensils shall be stored with handles above the top of the food, on a clean sanitized surface, in running water, or in water that is at least 135øF. Single-use articles and multiservice utensils shall be handled, displayed, and dispensed so that contamination of food and lip-contact surfaces is prevented. Extra preset tableware shall be removed when a consumer is seated or cleaned and sanitized before further use. Pressurized cylinders shall be securely fastened to rigid structure. Linens must be free of food residue and soil and be laundered as required.
