Conditions Observed: Measured foods in the top of the deli prep above 41 F (cheese, 48 F; shredded cheese, 49 F). Adjust/service/repair to cold-hold at 41 F or below.

“Except during preparation, cooking, cooling, transportation to or from a retail food facility for a period of less than thirty minutes, when time is used as the public health control, or as otherwise specified, potentially hazardous foods shall be maintained at or below 41F or at or above 135F. The following foods may be held at or below 45F: raw shell eggs, unshucked live molluscan shellfish, pasteurized milk and pasteurized milk products in original sealed containers, potentially hazardous foods held during transportation. Potentially hazardous foods held for dispensing in serving lines and salad bars may be held at 45F, not to exceed 12 hours in a 24 hour period, if unused portions are discarded. 10. Proper cooling methods.
Conditions Observed: Measured two hotel pans of rice above 50 F (rice, 50 F; rice, second pan, 52 F). Per Employee, rice was prepared yesterday. Rice shall be discarded. Reviewed rapid cooling. Measure cooling foods frequently using a metal stem thermometer. Avoid stacking/nesting pans used to cool food. Owner agrees to cook half the amount of rice. This violation requires a reinspection on/after 9/30/21.
ROACHY SAYS:
“After heating, potentially hazardous foods shall be rapidly cooled from 135F to 70F within 2 hours, and then from 70F to 41F within 4 hours. Potentially hazardous foods prepared from ambient temperature ingredients must be cooled to below 41F within 4 hours. Cooling shall be facilitated by one or more of the following methods: in shallow pans, separating food into smaller portions, adding ice as an ingredient, using an ice bath and stirring frequently, using an ice paddle, using rapid cooling equipment, or using containers that facilitate heat transfer. 12. Proper reheating procedures for hot holding.
Conditions Observed: Measured advance prep foods in the steam table below 135 F today (tofu in sauce, 115 F; sauce, 117 F). Reviewed reheating requirements with kitchen staff. Observed impacted foods were brought to the stove for further reheating. Throughly reheat foods to 165 F prior to hot holding; keep hot held foods at or above 135 F.
ROACHY SAYS:
“Potentinally hazardous food that is cooked, cooled, and reheated for hot holding shall be heated to a temperature of 165F for 15 seconds. Cooked and refrigerated food that is prepared for immediate service in response to an individual consumer order may be served at any temperature. Reheating for hot holding shall be done rapidly, and the time between 41F and 165F shall not exceed two hours. Potentinally hazardous food reheated in a microwave oven for hot holding shall be rotated or stirred midway during cooking, covered to retain surface moisture, be reheated to 165F in all parts of the food, and stand covered for at least two minutes after reheating. Ready-to-eat food taken from a commercailly processed hermetically sealed container or from an intact package from a food processing plant shall be heated to a temperature of 135F for hot holding. Conditions Observed: Observed black mildew like build-up in and around water tray in ice machine. Observed grey streaking, build-up on divider in ice machine. Unplug and empty ice machine; throughly clean using a mild soapy solution. Remove all debris and soap using clear water prior to sanitizing impacted surfaces. This major violation requires a reinspection on/after 9/30/21.

“Food-contact surfaces and utensils shall be clean to sight and touch. Food-contact surfaces and multiservice utensils shall be effectively washed, rinsed, and sanitized to remove or completely loosen soils by use of manual or mechanical methods. Precleaning may be required. Manual sanitization shall be accomplished during the final rinse by contact with solution containing 100 ppm available chlorine, 200 ppm quaternary ammonium, or another approved sanitizer. Mechanical sanitization shall be accomplished in the final rinse by achieving a utensil surface temperature of 160F or by contact with solution containing 50 ppm available chlorine, 200 ppm quaternary ammonium, or another approved sanitizer. After cleaning and sanitizing, equipment and utensils shall be air dried. Mechancial warewash machines must be approved and installed and operated according to manufacturer's specifications. Food contact surfaces, utensils, and equipment shall be cleaned and sanitized at the following times: before each use with different type of raw food of animal origin, when changing from raw food to ready to eat food, between uses with raw produce and potentially hazardous food, before using a thermometer, and any time during the operation when contamination may have occurred. Conditions Observed: Observed back door propped open at time of inspection. Observed screen door to be ill fitting. Keep door closed to maintain a vermin proof facility. Repair screen door to be tightly fitting.

Conditions Observed: Measured chicken in the steam table to be 128 F. Cook returned to stove, stirring and measuring chicken. Chicken was returned to the stream table when the center reached 165 F - COS. As discussed and observed during the inspection, foods can be boiling on the edge of the pan while the center of the food is much cooler. Always reheat foods to 165 F before placing in the steam table. Measured pinto beans in the left steam table to be 132 F. Cook stirred; measured pinto beans to be 138 F. COS. Revise hot holding method (i.e., stir often, keep foods covered) to keep all foods at 135 F or greater. Avoid stacking food above the top of the container.

Observed food debris, oily like build-up on two plates, a bowl, a soda cup. Showed observations to Owner. Maintain all utensils and tableware to be clean to the sight and clean to the touch. Remove heavily stained dishes from service. Observed minor, black mildew-like build-up on upper right corner of ice machine in storage area. Remove build-up; clean and sanitize.

Observed build-up on the prong of table mounted can opener. Observed brown, pink speckled build-up on front of divider in ice machine. Remove buidl-up; clean and sanitize impacted surfaces. Conditions Observed: Observed chicken in red sauce cooling in plastic containers. Measured chicken to be be 102 F, 114 F, and 82 F. Per Operator, chicken had been cooled in an ice bath. No cooling log available for review. Observed Cook pour chicken into hotel pans and put pans on ice. This method is more likely to achieve rapid cooling. Observed operator put pot of pinto beans in ice bath. Inspector measured pinto beans to 192 F. Employee return beans to stove top to rest. Measured some advance prep foods to be at 41 F in the walkin. Measured black beans cooling in an ice bath to 68 F at the end of the inspection. Revise your cooling method to use shallow, metal containers, ice wands and stirring, or a similar, approved method. See CalCode provided at the inspection. Use the cooling log provided today. You are required to email, fax, or otherwise provide a copy of your cooling log for each week to DEH by 4PM on Friday. Failure to do so will result in a billable reinspection.

Conditions Observed: Observed screen in ceiling above left steam table to be failing. Repair or replace to reduce the potential of physical contamination of foods or utensils.
ROACHY SAYS:
“Food facilities shall be fully enclosed in a building consisting of permanent floors, walls, and an overhead structure that meets minimum standards. The walls and ceiling shall be smooth, durable, nonabsorbent, and easily cleanable. Flooring and base coving shall be smooth, durable, and made of approved nonabsorbent material that is easily cleanable. Floor surfaces shall be coved at the juncture of the floor and wall with a 3/8 inch minimum radius and shall extend up the wall at least 4 inches.
