Conditions Observed: Observed employee slicing tomatoes directly from the box at the prep sink. Tomatoes did not appear to be washed. When asked, the employee indicated that tomatoes were not rinsed prior to cutting. Employee obtained a colander and rinse remaining tomatoes in the prep sink - COS. Always wash/rinse whole produce prior to cutting.

ROACHY SAYS:

“Produce shall be thoroughly washed in potable water to remove soil and other contaminants before being cut, combined with other ingredients, cooked, served, or offered for human consumption. Chemicals used to wash or peel produce shall meet the requirements specifice in 21 CFR 173.315. (113992)”

Conditions Observed: Observed excessive food debris in the cubbies used for pizza boxes and in lower left side of the flour cabinet at the dough station. Remove debris; maintain tidy surfaces.

Conditions Observed: No pump soap at the dough room handwashing sink. Keep all handwashing sinks fully stocked with pump soap and paper towels.

Conditions Observed: Observed tile flooring and associated coving to be failing in the dough room (e.g., below the handwash sink, lower left of the dough table). Grout has failed, is recessed between tiles. Repair or replace floor to be smooth, durable, non-absorbent, and free of harborage. Identify a more appropriate finish.

Food-contact surfaces and multiservice utensils shall be effectively washed, rinsed, and sanitized to remove or completely loosen soils by use of manual or mechanical methods. Precleaning may be required. Manual sanitization shall be accomplished during the final rinse by contact with solution containing 100 ppm available chlorine, 200 ppm quaternary ammonium, or another approved sanitizer. Mechanical sanitization shall be accomplished in the final rinse by achieving a utensil surface temperature of 160F or by contact with solution containing 50 ppm available chlorine, 200 ppm quaternary ammonium, or another approved sanitizer. After cleaning and sanitizing, equipment and utensils shall be air dried. Mechancial warewash machines must be approved and installed and operated according to manufacturer's specifications. Conditions Observed: Measured foods in the salad fridge/beer fridge to be above 41 F (cottage cheese, 45 F; dressing, 46 F). Adjust/service/repair to cold-hold foods at 41 F or below.

“Except during preparation, cooking, cooling, transportation to or from a retail food facility for a period of less than thirty minutes, when time is used as the public health control, or as otherwise specified, potentially hazardous foods shall be maintained at or below 41F or at or above 135F. The following foods may be held at or below 45F: raw shell eggs, unshucked live molluscan shellfish, pasteurized milk and pasteurized milk products in original sealed containers, potentially hazardous foods held during transportation. Potentially hazardous foods held for dispensing in serving lines and salad bars may be held at 45F, not to exceed 12 hours in a 24 hour period, if unused portions are discarded. Conditions Observed: Observed pink slime-like build-up on back tab in ice dispenser. Showed observations to PIC and reviewed cleaning-in-place. Remove debris; wash, rinse, and sanitze dispenser. Clean nightly to avoid build-up. PIC cleaned dispenser - COS.

Conditions Observed: Observed floor/tile to be failing around floor sink to right of walkin. Observed liquid pooling between tiles below wall mounted sanitizing station. Observed 4 by 6 inch gap in mens room wall to right of sink. Maintain floors and walls.

Conditions Observed: Observed excessive build-up staining on a knife and fork offered for use at the salad bar. Inspect utensils to ensure they are clean to the sight.

Conditions Observed: Observed 20 to 30 black, 0.5 cm droppings in far left corner of cabinet below dough table. Showed observations to Manager. Inspect flour and sugar containers for contamination. Remove droppings using a sanitizing solution, paper towels, and gloves. Wash hands after cleaning. Identify and seal vermin ingress points. This violation requires a reinspection on/after 3/28/19.
