Routine Inspection
02. Food safety certification, food handler card compliance

Conditions Observed: Facility lacks a Food Safety Manager (FSM). Obtain FSM certificate and maintain a copy at the facility for review during routine inspections.

ROACHY SAYS:

“Food facilities that prepare, handle, or serve non-prepackaged potentially hazardous food, shall have an owner or employee who has passed an aproved food safety certification examination. Food handlers that prepare, handle, or serve non-prepackaged potentially hazardous food, shall obtain a valid Food Handler Card within 30 days after date of hire. (113947.1-113947.5, 113948)”

10. Proper cooling methods.

Conditions Observed: Measured bowl of mashed potatoes (used for potato salad) 80F. Observed potatoes in a metal bowl, covered with plastic wrap, and stored on a prep table at room temperature. Per employees, the potatoes were prepared approximately 3 hours prior to the inspection. Potentially hazardous foods must be rapidly cooled from 135F - 70F within 2 hours and from 70F - 41F within 4 hours with use of ice baths, ice paddles with frequent stirring, ice as an ingredient, dividing large quantities into smaller portions, spreading thick foods into thin layers and placing in walk-in refrigerator or freezer, etc. Improperly cooled potatoes were discarded and facility will use food cooling log to document cooling process to ensure time/temperature parameters for rapid cooling are being met.

ROACHY SAYS:

“After heating, potentially hazardous foods shall be rapidly cooled from 135F to 70F within 2 hours, and then from 70F to 41F within 4 hours. Potentially hazardous foods prepared from ambient temperature ingredients must be cooled to below 41F within 4 hours. Cooling shall be facilitated by one or more of the following methods: in shallow pans, separating food into smaller portions, adding ice as an ingredient, using an ice bath and stirring frequently, using an ice paddle, using rapid cooling equipment, or using containers that facilitate heat transfer. (114002, 114002.1)”

22. Hot and Cold Water Available

Conditions Observed: Hot water measured 115F at 3 compartment sink. Adjust/repair/replace water heater to deliver hot water at or above 120F at all mixing faucets at all times.

ROACHY SAYS:

“An adequate, protected, pressurized, approved potable supply of hot and cold water shall be provided at all times. Hot water shall be supplied at a minimum temperature of 120F when measured from the faucet. The temperature of water provided to handwashing facilities shall be at least 100F. Handwashing facilities equipped with non-adjustable faucets shall have their temperature set between 100F and 108F. Hot water shall be provided at the food preparation sink and janitorial sink. 27. Approved thawing methods used; frozen food.

Conditions Observed: Observed raw meats thawing at room temperature. Use an approved method for thawing potentially hazardous foods.

ROACHY SAYS:

“Frozen foods shall be stored or displayed in their frozen state unless being thawed in the following manner: under refrigeration that maintains the food temperature at 41F or below, completely submerged under potable running water for a period not to exceed two hours at a water temperature of 70F or below and with sufficient water velocity to agitate and flush off loose particles into the sink drain, in a microwave oven if immediately followed by immediate preparation, or as part of a cooking process. (114018, 114020)”

28. Food separated and protected.

Conditions Observed: Observed tray of raw katsu chicken stored over ready-to-eat foods in bottom portion of cookline prep table. Store raw potentially hazardous foods of animal origin under ready-to-eat foods, based on final internal cooking temperatures, to prevent cross-contamination.

ROACHY SAYS:

“Food shall be protected from contamination during transportation, storage, preparation, holding, and display. Adequate and suitable counter space shall be provided for all food preparation. Food preparation shall be conducted wthin an approved food compartment or fully enclosed food facility. Open-air barbecues shall be separated from public access. 36. Equipment / Utensils approved; installed, clean, good repair, capacity.

Conditions Observed: Observed nonfunctional ice machine stored in the facility. Put ice machine back into service, or remove it from the premises. Observed missing (cold) faucet handle at 3 compartment sink. Repair faucet handle.

ROACHY SAYS:

“Utensils and equipment shall be kept fully operative and in good repair. All new and replacement equipment shall be properly installed and be certified or classified for sanitation by an ANSI accredited certification program. Electrical appliances must meet UL standards. Single-use articles shall not allow migration of deleterious substances or impart colors, odors, or tastes to food. Multi-use food contact surfaces must be smooth, free of breaks, chips, and similar imperfections; free of sharp internal angles, corners, and crevices; finished to have smooth welds and joints; be accessible for cleaning and inspection. Unless specified, wood and wood wicker may not be used as a food-contact surface. Unless specified, copper and copper alloys such as brass may not be used in contact with a food that has a pH below six or for a fitting between a backflow preventer and a carbonator. Beverage tubing and cold-plate beverage cooling devices cannot be installed in contact with stored ice intended to be used as food. Equipment that is not movable must be installed to allow for cleaning in place. Cutting or piercing parts of can openers must be readily removable. Facilities that wash, rinse, soak, thaw, or similarly prepare foods shall be provided with a approved food preparation sink. Reservoirs that supply water to produce foggers must be cleaned and maintained. Surfaces such as cutting blocks and boards that are subject to scratching and scoring shall be resurfaced or replaced if they can no longer be effectively cleaned and sanitized. 37. Equipment, utensils and linens: storage and use.

Conditions Observed: Observed utensil, used for portioning steamed rice, stored in a container of room temperature water. Store utensil in a clean, dry container and swap out the container and utensil at least every 4 hours, or store utensil in hot water maintained at/above 135F and swap out daily.

“Clean equipment, utensils, linens, and single-use articles shall be stored at least six inches above the floor in an approved location within the fully enclosed permitted food facility. Clean equipment, utensils, linens, and single-use articles shall not be exposed to splash, dust, vermin, or other forms of contamination. Non-food items shall be stored and displayed separate from food and food-contact surfaces. During pauses in food preparation utensils shall be stored with handles above the top of the food, on a clean sanitized surface, in running water, or in water that is at least 135øF. Single-use articles and multiservice utensils shall be handled, displayed, and dispensed so that contamination of food and lip-contact surfaces is prevented. Extra preset tableware shall be removed when a consumer is seated or cleaned and sanitized before further use. Pressurized cylinders shall be securely fastened to rigid structure. Linens must be free of food residue and soil and be laundered as required. 40. Wiping cloths: properly used and stored.

Conditions Observed: Observed wet wiping cloths stored on cutting board of food prep table on the cookline. You may use a wet wiping cloth once and then launder it, or use it multiple times if held in an approved sanitizer solution.

ROACHY SAYS:

“Wiping cloths used to clean food spills shall be used only once or stored in a sanitizing solution of an approved concentration as specified in section 114099.6. Dry or wet cloths that are used with raw foods of animal origin shall be kept separate from cloths used for other purposes, cloths that come into contact with raw foods of animal origin shall be kept in a separate sanitizing solution. Wiping cloths shall be free of food debris and visible soil. Working containers of sanitizing solutions for storage of in-use wiping cloths shall be used in a manner to prevent contamination of food, equipment, utensils, linens, or single-use articles. Wet wiping cloths shall be laundered daily. Dry wiping cloths shall be laundered as necessary to prevent contamination of food and clean serving utensils. (114135, 114185.1, 114185.3(d-e))”

44. Premises; personal/cleaning items; vermin-proofing.

Conditions Observed: Observed back door propped open over the duration of the inspection. Keep doors closed at all times to maintain a vermin proof facility, or install a screen to prevent entrance or harborage of animals, birds and vermin, including but not limited to insects and rodents.

ROACHY SAYS:

“All premises of a food facility shall be kept clean, fully operative, and in good repair. The premise of a food facility shall be free of litter and items unnecessary to the operation of the facility. Areas designated for employees to eat and drink shall be located so that food, clean equipent and utensils, linens, and single-use articles are protected from contamination. Personal items shall be stored in a designated area to prevent contamination of food or utensils. Food prep sinks, warewashing sinks, and handwashing sinks shall not be used for the cleaning or maintenance of tools or cleaning supplies or to dispose of mop water and similar liquid wastes. A dedicated janitorial sink with a drain shall be provided and conveniently located. A separate room, area, or cabinet shall be provided for cleaning supplies. The facility shall be kept vermin proof. Insect electrocution devices shall retain the insect within the device and shall not be located over food or utensil handling areas. Conditions Observed: Install a paper towel dispenser at BBQ handwash sink to protect paper towels from contamination.

ROACHY SAYS:

“Handwashing facilities shall be provided within or adjacent to toilet rooms, food preparation, and warewashing areas. Handwashing facilities shall be clean, unobstructed, and accessible at all times. Handwashing facilities shall be provided with soap and sanitary single use towels in approved dispensers. Dispensers shall be maintained in good repair. A handwashing facility shall not be used for purposes other than handwashing. (113953, 113953.1, 113953.2, 114067(f))”

28. Food separated and protected.

Conditions Observed: Observed raw chicken stored over raw pork in walk-in refrigerator. Store raw potentially hazardous foods of animal origin based on final internal cooking temperatures to prevent cross-contamination. Operator moved chicken to bottom shelf, this repeat minor violation was corrected.

35. Warewashing: installed, maintained, used, Adequate Means to measure sanitizer

Conditions Observed: Provide QT test strips to verify the concentration of the QT sanitizer solution is 200-400PPM.

ROACHY SAYS:

“Facilities that prepare food shall be equipped with a manual warewashing sink that has at least three compartments with two integral metal drainboards, except as specified in 114099(c). A warewashing sink shall not be used for handwashing except as specified in 114125(a). Sink compartments shall be large enough to accommodate immersion of the largest equipment and utensils used. Testing materials shall be provided to adequately measure the applicable sanitization method used during manual or mechanical warewashing. (114067(f,g), 114099, 114099.3, 114099.5, 114101(a), 114101.1, 114101.2, 114103, 114107, 114125(a))”

Conditions Observed: Observed utensil used to portion rice stored in room temperature water. If you choose to store the rice utensil in water, it must be held at/above 135F at all times. You may store the rice utensil in a clean container without water and change-out or wash, rinse and sanitize the utensil and the container at least every 4 hours. Operator provided a clean container without water for the storage of the rice utensil, this repeat minor violation was corrected.

Conditions Observed: Observed standing water in utility closet. Repair drain and remove standing water to prevent a water source for vermin.

“Steam tables, ice machines, ice bins, food preparation sinks, display cases, walk-in refrigeration units, and other similar equipment that discharge liquid waste shall drain into a floor sink with an air gap or other approved type of receptor. All plumbing and plumbing fixtures shall be installed in compliance with applicable local plumbing ordinances, maintained so as to prevent any contamination, kept clean, fully operative, and in good repair. The potable water supply shall be protected with a backflow or back siphonage protection device. Any hose used for potable water shall be of approved materials, labeled, properly stored, and used for no other purpose. 45. Floor, walls, and ceilings: built, maintained, clean.

Conditions Observed: Observed missing floor tiles and grout in walk-in refrigerator, adjacent to walk-in freezer door. Repair floor and grout to provide a surface that is smooth, durable, nonabsorbent and easy to clean.

ROACHY SAYS:

“Food facilities shall be fully enclosed in a building consisting of permanent floors, walls, and an overhead structure that meets minimum standards. The walls and ceiling shall be smooth, durable, nonabsorbent, and easily cleanable. Flooring and base coving shall be smooth, durable, and made of approved nonabsorbent material that is easily cleanable. Floor surfaces shall be coved at the juncture of the floor and wall with a 3/8 inch minimum radius and shall extend up the wall at least 4 inches. Routine Inspection
02. Food safety certification, food handler card compliance

Conditions Observed: On-site for routine inspection and reinspection for food handler cards that were not on file for review during the routine inspection that was conducted on 9/18/19, or today. Obtain copies of food handler cards for all employees and keep on file for review during routine inspections. Please submit copy of food handler cards to this office by 2/29/20 to avoid a billable reinspection in the amount of $133.

Store potentially hazardous foods, based on final internal cooking temperatures, to prevent cross-contamination. Observed raw meat in direct contact with plastic grocery bag in walk-in refrigerator. Store foods in commercial (NSF), food-grade containers to prevent contamination.

Conditions Observed: Observed unlabeled chemical spray bottle, containing QT sanitizer, stored behind cash register counter. Label all chemicals when taken out of manufacturer’s packaging for safety. Employee labeled spray bottle, this minor violation was corrected.

ROACHY SAYS:

“Only those insecticides, rodenticides, and other pesticides that are necessary, bear a legible manufacturer's label, and are specifically approved for use in a food facility may be used in a food facility in accordance with the manufacturer's instructions. Containers used for storing poisonous or toxic materiails such as cleaners and sanitizers taken from bulk supplies shall be clearly and individually identified with the common name of the material. Poisonous or toxic materials shall be stored or displayed to prevent contamination of food, equipment, utensils, linens, and single-use articles. (114254, 114254.1, 114254.2)”

Conditions Observed: Test strips were not available during the inspection. Obtain QT test strips, store in a readily available location, and use each time sanitizer is made to verify it is at the correct concentration of 200 - 400PPM.

Repair, replace or remove the ice machine from the facility.

Keep the door closed, or install a screen, to prevent entrance or harborage of animals, birds or vermin, including but not limited to, insects and rodents. Observed back door closed at end of the inspection, this minor violation was corrected. Observed cookline handwash sink is no longer sealed to the wall. Caulk the handwash sink to the wall to provide a smooth surface.

Routine Inspection
02. Food safety certification, food handler card compliance

Conditions Observed: Food handler cards were not available for review during the inspection. Obtain food handler cards for all employees within 30 days and submit copies to this office for verification by 10/18/19 to avoid a reinspection. Keep copies of all food handler cards on file at the facility for review during routine inspections.

ROACHY SAYS:

“Food facilities that prepare, handle, or serve non-prepackaged potentially hazardous food, shall have an owner or employee who has passed an aproved food safety certification examination. Food handlers that prepare, handle, or serve non-prepackaged potentially hazardous food, shall obtain a valid Food Handler Card within 30 days after date of hire. (113947.1-113947.5, 113948)”

07. Adequate handwash facilities supplied and accessible.

Conditions Observed: Observed outside handwash sink is dirty and the drain is clogged. Observed outside handwash station lacks required dispensers for pump soap and paper towels. Unclog the sink, maintain clean and unobstructed at all times for proper handwashing while using the BBQ** This sink is required for the operation of the open air BBQ. This is a major violation that will require a reinspection on or after 9/23/19.

ROACHY SAYS:

“Handwashing facilities shall be provided within or adjacent to toilet rooms, food preparation, and warewashing areas. Handwashing facilities shall be clean, unobstructed, and accessible at all times. Handwashing facilities shall be provided with soap and sanitary single use towels in approved dispensers. Dispensers shall be maintained in good repair. A handwashing facility shall not be used for purposes other than handwashing. (113953, 113953.1, 113953.2, 114067(f))”

08. Proper hot and cold holding temperatures.

Conditions Observed: Measured all items in cookline prep table, top and bottom portions >50F with probe thermometer (see measured observations). Repair/replace this unit to hold cold potentially hazardous foods at or below 41F at all times. All potato salad, macaroni salad, and other potentially hazardous food items were discarded. Cut vegetables, prepped approximately 1/2 hour ago were transferred to the walk-in refrigerator. You may set up an ice bath to keep foods under temperature control at the cookline if you purchase an abundance of commercially pre-bagged ice and monitor and log holding temperatures of foods held in the ice bath at least 2x/day; which is to be observed/verified during the reinspection. This is a major violation that will require a reinspection on or after 9/23/19.

ROACHY SAYS:

“Except during preparation, cooking, cooling, transportation to or from a retail food facility for a period of less than thirty minutes, when time is used as the public health control, or as otherwise specified, potentially hazardous foods shall be maintained at or below 41F or at or above 135F. The following foods may be held at or below 45F: raw shell eggs, unshucked live molluscan shellfish, pasteurized milk and pasteurized milk products in original sealed containers, potentially hazardous foods held during transportation. Potentially hazardous foods held for dispensing in serving lines and salad bars may be held at 45F, not to exceed 12 hours in a 24 hour period, if unused portions are discarded. Food preparation shall not exceed two cumulative hours without a return to the required holding temperatures. (113996, 113998, 114037, 114343(a))”

24. No rodents, insects, birds, or animals.

Conditions Observed: Observed excessive amount of flies in kitchen area. Flies are a vermin that have the potential to cross-contaminate. Increase pest control and cleaning efforts to remove flies from the facility. Keep doors closed at all times. Recommend obtaining insect electrocution devices that retain the insect bodies, install away from food and utensil storage and handling areas. This is a major violation that will require a reinspection on or after 9/23/19.

ROACHY SAYS:

“A food facility shall be kept free of vermin. Live animals are prohibited from food facilities except as specified in 114259.5. Food facility employees shall not care for or handle animals that may be present in the food facility. Food facility employees are required to wash their hands after handling or caring for fish in an aquarium and/or shellfish, or crustacea, in display tanks and as specified in 114259.4. (114259.1, 114259.4, 114259.5)”

28. Food separated and protected.

Conditions Observed: Observed open bulk bags of food on wire rack. Once a bulk bag of food is opened, transfer it to a rigid container with a tight fitting lid to protect from contamination or potential vermin attraction. Observed personal food items stored in the walk-in refrigerator. Completely segregate personal items from items used for retail food sales for safety. Observed raw eggs stored over produce in walk-in refrigerator. Store raw potentially hazardous food items under ready-to-eat items to prevent cross-contamination.

“Food shall be protected from contamination during transportation, storage, preparation, holding, and display. Adequate and suitable counter space shall be provided for all food preparation. 30. Toxic substances properly identified, stored, used.

Conditions Observed: Observed unlabeled chemical spray bottle adjacent to handwash station behind cash register. Label all chemicals when taken out of original manufacturer’s packaging for safety.

ROACHY SAYS:

“Only those insecticides, rodenticides, and other pesticides that are necessary, bear a legible manufacturer's label, and are specifically approved for use in a food facility may be used in a food facility in accordance with the manufacturer's instructions. Containers used for storing poisonous or toxic materiails such as cleaners and sanitizers taken from bulk supplies shall be clearly and individually identified with the common name of the material. Poisonous or toxic materials shall be stored or displayed to prevent contamination of food, equipment, utensils, linens, and single-use articles. (114254, 114254.1, 114254.2)”

31. Food storage; food storage containers identified.

Conditions Observed: Observed food stored on the floor in walk-in refrigerator and walk-in freezer. Store foods at least 6 inches above the floor to prevent contamination or potential vermin attraction.

ROACHY SAYS:

“Adequate and suitable space shall be provided for the storage of food. Food shall be protected from contamination by storing the food in a clean, dry location, where it is not exposed to splash, dust, vermin, or other forms of contamination or adulteration, and at least six inches above the floor. Food shall not be stored in any of the following ways: in locker rooms, toilet rooms, dressing rooms, refuse rooms, mechanical rooms, under sewer lines that are not shielded to intercept potential drips, under leaking water lines, including leaking automatic fire sprinklers heads, or under lines on which water has condensated, under open stairwells or under sources of contamination. Working containers holding food or food ingredients that are removed from their original packages for use in the food facility shall be identified with the common name of the food. Non-prepackaged food may not be stored in direct contact with undrained ice. Products that are held by the permit holder for credit, redemption, or return to the distributor, such as damaged, spoiled, or recalled products shall be segregated and held in designated areas that are separated from food, equipment, utensils, linens, and single-use articles. 35. Warewashing: installed, maintained, used, Adequate Means to measure sanitizer

Conditions Observed: QT test strips were not available during the inspection. Obtain QT test strips, store in a readily available location and use each time QT sanitizer is made to verify it is at the correct concentration of 200PPM.

ROACHY SAYS:

“Facilities that prepare food shall be equipped with a manual warewashing sink that has at least three compartments with two integral metal drainboards, except as specified in 114099(c). A warewashing sink shall not be used for handwashing except as specified in 114125(a). Sink compartments shall be large enough to accommodate immersion of the largest equipment and utensils used. Testing materials shall be provided to adequately measure the applicable sanitization method used during manual or mechanical warewashing. 36. Equipment / Utensils approved; installed, clean, good repair, capacity.

Conditions Observed: Observed styrofoam cup used to portion coleslaw in prep table. Obtain an approved utensil, with a handle and store with handle facing up out of contact with food to prevent cross-contamination.

ROACHY SAYS:

“Utensils and equipment shall be kept fully operative and in good repair. All new and replacement equipment shall be properly installed and be certified or classified for sanitation by an ANSI accredited certification program. Electrical appliances must meet UL standards. Single-use articles shall not allow migration of deleterious substances or impart colors, odors, or tastes to food. Multi-use food contact surfaces must be smooth, free of breaks, chips, and similar imperfections; free of sharp internal angles, corners, and crevices; finished to have smooth welds and joints; be accessible for cleaning and inspection. Unless specified, wood and wood wicker may not be used as a food-contact surface. Unless specified, copper and copper alloys such as brass may not be used in contact with a food that has a pH below six or for a fitting between a backflow preventer and a carbonator. Beverage tubing and cold-plate beverage cooling devices cannot be installed in contact with stored ice intended to be used as food. Equipment that is not movable must be installed to allow for cleaning in place. Cutting or piercing parts of can openers must be readily removable. Facilities that wash, rinse, soak, thaw, or similarly prepare foods shall be provided with a approved food preparation sink. Reservoirs that supply water to produce foggers must be cleaned and maintained. 37. Equipment, utensils and linens: storage and use.

Conditions Observed: Observed utensils used to portion rice stored in room temperature water. The rice utensils must be stored in hot water at or above 135F at all times to prevent growth of bacteria. Provide proper storage for the rice utensil between use.

ROACHY SAYS:

“Clean equipment, utensils, linens, and single-use articles shall be stored at least six inches above the floor in an approved location within the fully enclosed permitted food facility. Clean equipment, utensils, linens, and single-use articles shall not be exposed to splash, dust, vermin, or other forms of contamination. Non-food items shall be stored and displayed separate from food and food-contact surfaces. During pauses in food preparation utensils shall be stored with handles above the top of the food, on a clean sanitized surface, in running water, or in water that is at least 135øF. Single-use articles and multiservice utensils shall be handled, displayed, and dispensed so that contamination of food and lip-contact surfaces is prevented. Extra preset tableware shall be removed when a consumer is seated or cleaned and sanitized before further use. Pressurized cylinders shall be securely fastened to rigid structure. Linens must be free of food residue and soil and be laundered as required. 38. Adequate ventilation and lighting, designated areas, use.

Conditions Observed: Observed grease accumulation on ventilation hood and ventilation hood baffle filters. Clean and sanitize hood baffle filters regularly to increase air flow and prevent a fire hazard. The ventilation hood and duct must be professionally cleaned and hood sticker or service invoice provided for verifcation.

ROACHY SAYS:

“Approved exhaust hoods shall be provided to remove toxic gases, heat, grease, vapors and smoke. Canopy-type hoods shall extend six inches beyond all cooking equipment. Toilet rooms shall be consistent with local building codes and vented to the outside air by a screened openable window, air shaft, or light-switch activated exhaust fan. Adequate lighting shall be provided in all areas to facilitate cleaning and inspection. Light fixtures in areas where open food is stored, served, prepared, and where utensils are washed shall be of shatterproof construction or protected with light shields. 40. Wiping cloths: properly used and stored.

Conditions Observed: Observed soiled wiping cloths held out of approved sanitizer solution. You may use a wiping cloth once and then launder it, or use it multiple times if held in an approved sanitizer solution. *Clean and sanitized utensils must be air dried! Do not use wiping cloths to line the drain board when air drying utensils.

ROACHY SAYS:

“Wiping cloths used to clean food spills shall be used only once or stored in a sanitizing solution of an approved concentration as specified in section 114099.6. Dry or wet cloths that are used with raw foods of animal origin shall be kept separate from cloths used for other purposes, cloths that come into contact with raw foods of animal origin shall be kept in a separate sanitizing solution. Wiping cloths shall be free of food debris and visible soil. Working containers of sanitizing solutions for storage of in-use wiping cloths shall be used in a manner to prevent contamination of food, equipment, utensils, linens, or single-use articles. Wet wiping cloths shall be laundered daily. Dry wiping cloths shall be laundered as necessary to prevent contamination of food and clean serving utensils. (114135, 114185.1, 114185.3(d-e))”

41. Plumbing: proper backflow devices.

Conditions Observed: Observed standing water and clogged floor drain in utility closet. Repair.

ROACHY SAYS:

“Steam tables, ice machines, ice bins, food preparation sinks, display cases, walk-in refrigeration units, and other similar equipment that discharge liquid waste shall drain into a floor sink with an air gap or other approved type of receptor. All plumbing and plumbing fixtures shall be installed in compliance with applicable local plumbing ordinances, maintained so as to prevent any contamination, kept clean, fully operative, and in good repair. The potable water supply shall be protected with a backflow or back siphonage protection device. Any hose used for potable water shall be of approved materials, labeled, properly stored, and used for no other purpose. 43. Toilet facilities: properly constructed, supplied, cleaned.

Conditions Observed: Observed restrooms are lacking self-closing doors. Install self-closing mechanisms on restroom doors to prevent passage of odors, dust or insects.

“Toilet facilities shall be maintained and kept clean. Toilet rooms shall be separated by a well-fitting, self-closing door. Toilet tissue shall be provided in a permanently installed dispenser at each toilet. (114250, 114250.1, 114276)”

44. Premises; personal/cleaning items; vermin-proofing.

Conditions Observed: OBserved personal items (sweater) hanging on wire shelving unit containing food items. Store personal items in an area where contamination of food, equipment, utensils, linens and single-use articles cannot occur. Observed cookline handwash sink is not sealed to the wall. Caulk the handwash sink to the wall to provide a smooth surface. Observed back door and side door of the facility propped open during the inspection, resulting in Major violation #24. The facility is to remain vermin proof at all times. Keep doors closed, or install screens to prevent entrance or harborage of animals, birds or vermin, including but not limited to, insects and rodents.

“All premises of a food facility shall be kept clean, fully operative, and in good repair. The premise of a food facility shall be free of litter and items unnecessary to the operation of the facility. Areas designated for employees to eat and drink shall be located so that food, clean equipent and utensils, linens, and single-use articles are protected from contamination. Personal items shall be stored in a designated area to prevent contamination of food or utensils. Food prep sinks, warewashing sinks, and handwashing sinks shall not be used for the cleaning or maintenance of tools or cleaning supplies or to dispose of mop water and similar liquid wastes. A dedicated janitorial sink with a drain shall be provided and conveniently located. A separate room, area, or cabinet shall be provided for cleaning supplies. The facility shall be kept vermin proof. Insect electrocution devices shall retain the insect within the device and shall not be located over food or utensil handling areas. Conditions Observed: Observed failed floor in walk-in refrigerator adjacent to walk-in freezer. Repair floor to provide a surface that is smooth, durable, nonabsorbent and easy to clean. Observed mold-like substance on ceiling over warewash area. Clean and sanitize impacted area to prevent mold growth.

ROACHY SAYS:

“Food facilities shall be fully enclosed in a building consisting of permanent floors, walls, and an overhead structure that meets minimum standards. The walls and ceiling shall be smooth, durable, nonabsorbent, and easily cleanable. Conditions Observed: Observed box of chicken stored over boxes of pork in walk-in refrigerator. Store raw potentially hazardous foods based on final internal cooking temperatures to protect from cross-contamination. The whole cuts of beef and pork have a final internal cooking temperature of 145F, whereas chicken has a final internal cooking temperature of 165F. Therefore, the chicken must be stored under the beef and pork. Observed open bulk bags of food on wire rack. Once an open bag of food is opened, transfer it to a rigid container with a tight fitting lid to protect from contamination and/or vermin attractant. Observed food stored in plastic garbage bags and plastic grocery bags in walk-in freezer. Store food in food grade containers only to protect from adulteration.
