01. Demonstration of knowledge.
Conditions Observed: Facility is lacking a food safety manager (FSM). Obtain FSM certificate within 60 days and make available for review during routine inspections.
ROACHY SAYS:
“The Person In Charge (PIC) and all employees shall have adequate knowledge and shall be properly trained in food safety as it relates to their assigned duties. The PIC shall have adequate knowledge of major food allergens, foods identified as major food allergens, and the symptoms that a major food allergen could cause. The PIC means a designated person who has knowledge of safe food handling practices and the major food allergens as they relate to the specific food preparation activities that occur at the food facility. 06. Hands clean and properly washed; gloves used properly.
Conditions Observed: Observed cookline employee don a single-use glove to put raw chicken on the grill, then remove glove and attempt to don another glove without handwashing. Instructed employee on handwashing and proper glove use; hands must be washed prior to donning new gloves and hands must be washed after handling raw meat. Employee demonstrated active handwashing and proper glove use over the remainder of the inspection, this major violation was corrected. Use the handout that was provided to train staff.

Whenever gloves are worn, they shall be changed, replaced, or washed as often as handwashing is required. Single-use gloves shall not be washed. Conditions Observed: Observed paper towels not stocked in paper towel dispenser in employee restroom. Keep required supplies stocked in dispensers at all times for handwashing.

Conditions Observed: Observed approximately 6-8 flies hovering in the dining area and approximately 5-6 flies hovering in the rear food prep area. Observed uncovered recyclables and dust pan filled with a mixture of moisture and food debris near back door which may be attracting insects. Increase cleaning and professional pest control efforts to remove flies from the facility.

07. Adequate handwash facilities supplied and accessible.
Conditions Observed: The handwash sink was being used to thaw frozen shrimp. The handwash sink must be dedicated only for handwashing. Keep handwash sink clean, unobstructed and stocked with required supplies for handwashing at all times. Thawing frozen foods under running water must only be conducted at the food preparation sink. Employee moved frozen shrimp to the refrigerator and sanitized the handwash sink as instructed, thank you. The paper towel dispenser in the employee restroom is broken and paper towels are hanging on a chair. Repair/replace paper towel dispenser and keep it stocked to prevent contamination to paper towels used for handwashing.

Conditions Observed: Observed line cook don gloves, put raw ground beef patty on grill, then remove gloves and conduct food prep without required handwashing prior to donning gloves and without handwashing after handling raw potentially hazardous food from animal origin. Instructed line cook to wash hands after handling raw ground beef. Handwashing must be conducted prior to donning gloves and after handling raw meat to prevent cross-contamination. This is a repeat major violation that will require a reinspection on or after 12/17/19.

Conditions Observed: Measured sliced green onions 45.5F and sliced onions 45F with probe thermometer in cookline ice baths. Measured shredded pepper jack cheese 44.5F and basil pesto 45F with probe thermometer in Hobart refrigerator. Replenish ice in ice baths regularly to maintain temperatures at or below 41F at all times. Adjust/repair/replace Hobart to maintain temperatures at or below 41F at all times.
ROACHY SAYS:
“Except during preparation, cooking, cooling, transportation to or from a retail food facility for a period of less than thirty minutes, when time is used as the public health control, or as otherwise specified, potentially hazardous foods shall be maintained at or below 41F or at or above 135F. The following foods may be held at or below 45F: raw shell eggs, unshucked live molluscan shellfish, pasteurized milk and pasteurized milk products in original sealed containers, potentially hazardous foods held during transportation. Potentially hazardous foods held for dispensing in serving lines and salad bars may be held at 45F, not to exceed 12 hours in a 24 hour period, if unused portions are discarded. Conditions Observed: Observed mold-like substance in ice machine. Clean and sanitize ice machine per manufacturer’s instructions to prevent mold growth. This is a major violation that will require a reinspection on or after 12/17/19.
ROACHY SAYS:
“Food-contact surfaces and utensils shall be clean to sight and touch. Food-contact surfaces and multiservice utensils shall be effectively washed, rinsed, and sanitized to remove or completely loosen soils by use of manual or mechanical methods. Precleaning may be required. Manual sanitization shall be accomplished during the final rinse by contact with solution containing 100 ppm available chlorine, 200 ppm quaternary ammonium, or another approved sanitizer. Mechanical sanitization shall be accomplished in the final rinse by achieving a utensil surface temperature of 160F or by contact with solution containing 50 ppm available chlorine, 200 ppm quaternary ammonium, or another approved sanitizer. After cleaning and sanitizing, equipment and utensils shall be air dried. Mechancial warewash machines must be approved and installed and operated according to manufacturer's specifications. Food contact surfaces, utensils, and equipment shall be cleaned and sanitized at the following times: before each use with different type of raw food of animal origin, when changing from raw food to ready to eat food, between uses with raw produce and potentially hazardous food, before using a thermometer, and any time during the operation when contamination may have occurred. Warewash sinks used to wash wiping cloths, wash produce, or thaw food must be cleaned and sanitized before and after these uses. Equipment, food contact surfaces, and utensils must be cleaned and sanitized throughout the day at least every four hours or as needed to prevent contamination. “Only those insecticides, rodenticides, and other pesticides that are necessary, bear a legible manufacturer's label, and are specifically approved for use in a food facility may be used in a food facility in accordance with the manufacturer's instructions. Containers used for storing poisonous or toxic materiails such as cleaners and sanitizers taken from bulk supplies shall be clearly and individually identified with the common name of the material. Poisonous or toxic materials shall be stored or displayed to prevent contamination of food, equipment, utensils, linens, and single-use articles. (114254, 114254.1, 114254.2)”

Conditions Observed: Observed single-use plastic cup used to portion shredded cheese and single-use paper cup used to portion lettuce in True 3 door prep table. Obtain approved utensils with handles that are smooth, durable, nonabsorbent and easy to clean. Or, if you may use single-use utensils if they are discarded after each use.
ROACHY SAYS:
“Utensils and equipment shall be kept fully operative and in good repair. All new and replacement equipment shall be properly installed and be certified or classified for sanitation by an ANSI accredited certification program. Electrical appliances must meet UL standards. Single-use articles shall not allow migration of deleterious substances or impart colors, odors, or tastes to food. Multi-use food contact surfaces must be smooth, free of breaks, chips, and similar imperfections; free of sharp internal angles, corners, and crevices; finished to have smooth welds and joints; be accessible for cleaning and inspection. Unless specified, wood and wood wicker may not be used as a food-contact surface. Unless specified, copper and copper alloys such as brass may not be used in contact with a food that has a pH below six or for a fitting between a backflow preventer and a carbonator. Beverage tubing and cold-plate beverage cooling devices cannot be installed in contact with stored ice intended to be used as food. Equipment that is not movable must be installed to allow for cleaning in place. Cutting or piercing parts of can openers must be readily removable. Conditions Observed: Observed utensil handle in direct contact with bulk flour. Store utensils facing up out of contact with foods to prevent cross-contamination. Measured hot water 94.5F for utensil storage on the stove. Increase water temperature to at least 135F for utensil storage to prohibit bacteria growth. Observed clean utensils stored in soiled containers on prep table. Clean and sanitize containers regularly or cover the containers to prevent buildup.
ROACHY SAYS:
“Clean equipment, utensils, linens, and single-use articles shall be stored at least six inches above the floor in an approved location within the fully enclosed permitted food facility. Clean equipment, utensils, linens, and single-use articles shall not be exposed to splash, dust, vermin, or other forms of contamination. Non-food items shall be stored and displayed separate from food and food-contact surfaces. During pauses in food preparation utensils shall be stored with handles above the top of the food, on a clean sanitized surface, in running water, or in water that is at least 135øF. Single-use articles and multiservice utensils shall be handled, displayed, and dispensed so that contamination of food and lip-contact surfaces is prevented. Extra preset tableware shall be removed when a consumer is seated or cleaned and sanitized before further use. Pressurized cylinders shall be securely fastened to rigid structure. “Approved exhaust hoods shall be provided to remove toxic gases, heat, grease, vapors and smoke. Canopy-type hoods shall extend six inches beyond all cooking equipment. Toilet rooms shall be consistent with local building codes and vented to the outside air by a screened openable window, air shaft, or light-switch activated exhaust fan. Adequate lighting shall be provided in all areas to facilitate cleaning and inspection. Light fixtures in areas where open food is stored, served, prepared, and where utensils are washed shall be of shatterproof construction or protected with light shields. Conditions Observed: Observed back door propped open over the duration of the inspection. The facility is to remain vermin proof at all times. Keep back door closed or install a screen to prevent entrance or harborage of animals, birds or vermin, including but not limited to insects and rodents. Observed dead mouse on glue trap behind water heater. Remove dead mouse from premises. Clean and sanitize impacted area.

“All premises of a food facility shall be kept clean, fully operative, and in good repair. The premise of a food facility shall be free of litter and items unnecessary to the operation of the facility. Areas designated for employees to eat and drink shall be located so that food, clean equipent and utensils, linens, and single-use articles are protected from contamination. Conditions Observed: Observed cookline employee use gloved hand to put raw shrimp skewer on the grill. The employee then removed one glove and replaced with a new glove and failed to wash their hands. Handwashing is required after handling raw potentially foods from animal origin and prior to donning gloves. Use the handout that was provided to hold a training on handwashing, proper glove use and minimizing bare hand contact with ready to eat foods with all food handlers. Submit an employee sign-in sheet to this office for verification by 6/3/19 to avoid a reinspection.
ROACHY SAYS:
“Food employees shall minimize bare hand and arm contact with nonprepackaged food that is in ready to eat form. Conditions Observed: Observed handwash sink obstructed with utensils. The handwashing sink shall be maintained clean and unobstructed for handwashing at all times. Conditions Observed: Measured garlic oil 57F with probe thermometer in ice bath. Observed ice in ice bath had melted. If you are using ice baths, ensure ice is added throughout the day to maintain temperatures at or below 41F at all times. Employee replenished ice bath. Thank you for correcting this major violation. Measured salsa 45.5F, pesto 45F, gravy 44F and veggie burger 46F with probe thermometer in True 2 door prep table. Adjust/repair/replace True 2 door prep table to hold cold potentially hazardous foods at or below 41F at all times.

Conditions Observed: Observed mold-like substance in ice machine. Clean and sanitize ice machine per manufacturer’s instructions to prevent mold growth.

“Food-contact surfaces and utensils shall be clean to sight and touch. Food-contact surfaces and multiservice utensils shall be effectively washed, rinsed, and sanitized to remove or completely loosen soils by use of manual or mechanical methods. Precleaning may be required. Manual sanitization shall be accomplished during the final rinse by contact with solution containing 100 ppm available chlorine, 200 ppm quaternary ammonium, or another approved sanitizer. Mechanical sanitization shall be accomplished in the final rinse by achieving a utensil surface temperature of 160F or by contact with solution containing 50 ppm available chlorine, 200 ppm quaternary ammonium, or another approved sanitizer. After cleaning and sanitizing, equipment and utensils shall be air dried. Mechancial warewash machines must be approved and installed and operated according to manufacturer's specifications. 30. Toxic substances properly identified, stored, used.
Conditions Observed: Observed unlabeled spray bottled on wire rack in chemical storage area. Label all chemicals when taken out of original manufacturer’s packaging for safety. Observed unapproved chemical (Raid) under the ice bin in the bar. Raid is not approved for use in a food facility. Remove Raid from the facility and contact professional pest control for assistance with vermin issues.

“Only those insecticides, rodenticides, and other pesticides that are necessary, bear a legible manufacturer's label, and are specifically approved for use in a food facility may be used in a food facility in accordance with the manufacturer's instructions. Conditions Observed: Observed unapproved utensils used to portion potentially hazardous foods (salad, slaw, cilantro, etc.) in prep tables. Obtain approved utensils, with handles, and store with handles facing up out of contact with food to prevent cross-contamination.

“Utensils and equipment shall be kept fully operative and in good repair. All new and replacement equipment shall be properly installed and be certified or classified for sanitation by an ANSI accredited certification program. Electrical appliances must meet UL standards. Single-use articles shall not allow migration of deleterious substances or impart colors, odors, or tastes to food. Multi-use food contact surfaces must be smooth, free of breaks, chips, and similar imperfections; free of sharp internal angles, corners, and crevices; finished to have smooth welds and joints; be accessible for cleaning and inspection. Conditions Observed: Observed clean knives and utensils stored in soiled containers. Utensils shall be stored in sanitary containers to prevent contamination. Observed napkins, plastic souffle cups and mussel pots stored in employee restroom. These items should be stored in a sanitary location outside of the restroom.

Repair/replace floors to provide a smooth surface that is durable, nonabsorbent and easy to clean. Clean and sanitize floors regularly to prevent buildup.

Conditions Observed: Cookline was lacking a sanitizer bucket during food prep. Ensure you have a way to sanitize food-contact surfaces when you are prepping and cooking food, especially when working with raw potentially hazardous foods of animal origin. Employee set up QT sanitizer solution in sanitizer bucket that measured 200ppm. This major violation was corrected on site, thank you.

Conditions Observed: Observed rodent feces in utility room and around water heater. Deep clean and sanitize impacted areas. Highly recommend contacting a professional pest control service to identify and repair ingress holes, as well as identify and trap/remove rodents from facility. This is a repeat major violation that will require a reinspection on or after Tuesday 12/18/18.

Conditions Observed: Observed one non-functioning light under ventilation hood. Replace light bulb to provide adequate lighting. Observed service sticker for ventilation hood, “next service 11/18”. Observed grease accumulation on light fixtures and ansel system under ventilation hood. The ventilation hood shall be cleaned by a professional company at least 2x/year to increase air flow and prevent a fire hazard.
ROACHY SAYS:
“Approved exhaust hoods shall be provided to remove toxic gases, heat, grease, vapors and smoke. Canopy-type hoods shall extend six inches beyond all cooking equipment. Toilet rooms shall be consistent with local building codes and vented to the outside air by a screened openable window, air shaft, or light-switch activated exhaust fan.
