Conditions Observed: Observed mildew like debris on the back tab behind the ice dispenser. Observed water pooling in front of trash bin. Maintain tidy non-food contact surfaces.

Conditions Observed: Facility continues to lack a conforming food prep sink; observed sealed deli meats thawing in the 3-bin of the 3-part sink. As discussed at the December 2020 change-of-ownership inspection, your are required to install a food prep sink. Provide plans to this office prior to 11/7/21.

Please install, sink shall drain indirectly into a floor sink with a 1 inch air gap between drain and floor sink rim.

Conditions Observed: Observed 10 to 12 black, dry small (0.25 cm) droppings around base of water heater. Observed 5 to 7 similar droppings in, near debris on cabinet below soda fountain. Carefully inspect single-use items (lids and straws) in cabinet for contamination. Remove all droppings using a 10:1 bleach solution and the appropriate PPE. Increase pest control to remove vermin from the facility. Showed observations to PIC. This violation requires a reinspection on/after 1/29/2020. DEH recommends obtaining professional pest control.
ROACHY SAYS:

Conditions Observed: Our records show that you do not have a valid Food Safety Manager Certification on file with our office, and no certificate was provided today. Prior to 6/28/19, please mail, fax, or email a copy of a valid Certificate to our office.

Throughly clean ice dispenser; sanitize impacted surfaces. Apply sanitizer to a clean surface. This repeat violation requires a reinspection on/after 5/2/19.
