Conditions Observed: Observed unlabeled spray bottle, containing chlorine (bleach), stored under the 3 compartment sink. Label all chemicals when taken out of original manufacturer’s packaging for safety.

“Only those insecticides, rodenticides, and other pesticides that are necessary, bear a legible manufacturer's label, and are specifically approved for use in a food facility may be used in a food facility in accordance with the manufacturer's instructions. Conditions Observed: Observed buildup on non-food contact surface of the milkshake machine, on top of the grease container, on the soda machine near the dispensers, and on the interior door of the ice machine. Maintain non-food contact surfaces clean and free of buildup.

Conditions Observed: Observed obstructed handwash station in employee restroom. Keep handwash stations clean and unobstructed for handwashing at all times.

Conditions Observed: Observed canned foods and a bag of onions stored on the floor in dry storage area. Store foods at least 6 inches above the floor to prevent contamination to foods or a potential food source for vermin.

Food shall be protected from contamination by storing the food in a clean, dry location, where it is not exposed to splash, dust, vermin, or other forms of contamination or adulteration, and at least six inches above the floor. Food shall not be stored in any of the following ways: in locker rooms, toilet rooms, dressing rooms, refuse rooms, mechanical rooms, under sewer lines that are not shielded to intercept potential drips, under leaking water lines, including leaking automatic fire sprinklers heads, or under lines on which water has condensated, under open stairwells or under sources of contamination. Working containers holding food or food ingredients that are removed from their original packages for use in the food facility shall be identified with the common name of the food. Conditions Observed: Observed grease accumulation and food debris on floors under cookline equipment. Maintain floors clean to prevent buildup and potential vermin attraction. Floors are failing throughout the facility. Replace floors to provide a smooth surface that is durable, nonabsorbent and easy to clean with a minimum 3/8 inch radius coving that extends up the wall at least 4 inches. Submit spec sheets or floor samples to this office for approval prior to purchase and installation.

Conditions Observed: Measured ranch dressing 44F with probe thermometer in non-commercial Sanyo refrigerator. Replace this refrigerator with a commercial unit capable of holding potentially hazardous foods at or below 41F at all times. Submit spec sheets to this office for approval prior to purchase and installation.

“Except during preparation, cooking, cooling, transportation to or from a retail food facility for a period of less than thirty minutes, when time is used as the public health control, or as otherwise specified, potentially hazardous foods shall be maintained at or below 41F or at or above 135F. The following foods may be held at or below 45F: raw shell eggs, unshucked live molluscan shellfish, pasteurized milk and pasteurized milk products in original sealed containers, potentially hazardous foods held during transportation. Potentially hazardous foods held for dispensing in serving lines and salad bars may be held at 45F, not to exceed 12 hours in a 24 hour period, if unused portions are discarded. Conditions Observed: Measured hot water 117F with probe thermometer at 3 compartment sink. Please fill out hot water demands worksheet to see if the water heater supplies enough hot water for the establishment. Hot water shall be delivered at or above 120F at all mixing faucets at all times.

“An adequate, protected, pressurized, approved potable supply of hot and cold water shall be provided at all times. Hot water shall be supplied at a minimum temperature of 120F when measured from the faucet. The temperature of water provided to handwashing facilities shall be at least 100F. Handwashing facilities equipped with non-adjustable faucets shall have their temperature set between 100F and 108F. Hot water shall be provided at the food preparation sink and janitorial sink. Conditions Observed: Observed nonfunctional lights throughout facility. Repair/replace bulbs to provide adequate lighting.

Toilet rooms shall be consistent with local building codes and vented to the outside air by a screened openable window, air shaft, or light-switch activated exhaust fan. Adequate lighting shall be provided in all areas to facilitate cleaning and inspection. Light fixtures in areas where open food is stored, served, prepared, and where utensils are washed shall be of shatterproof construction or protected with light shields. All plumbing and plumbing fixtures shall be installed in compliance with applicable local plumbing ordinances, maintained so as to prevent any contamination, kept clean, fully operative, and in good repair. The potable water supply shall be protected with a backflow or back siphonage protection device. Any hose used for potable water shall be of approved materials, labeled, properly stored, and used for no other purpose. Conditions Observed: Observed 3 compartment sink is no longer secured to the wall. Repair sink to provide a smooth surface. Observed grease accumulation on grease container. Maintain grease container and surrounding area clean to prevent vermin attraction.

Conditions Observed: Observed gnats in employee restroom and surrounding area. Increase cleaning and pest control efforts to remove gnats from the facility.

Food facility employees shall not care for or handle animals that may be present in the food facility. Conditions Observed: Observed mold-like substance in ice machine interior. Clean and sanitize ice machine per manufacturer’s instructions on interior door and maintain this piece of equipment to prevent mold growth. Observed debris in floor sinks (under 3 compartment sink & under soda machine. Clean and sanitize floor sinks regularly to prevent buildup.

ROACHY SAYS:

“Utensils and equipment shall be kept fully operative and in good repair. All new and replacement equipment shall be properly installed and be certified or classified for sanitation by an ANSI accredited certification program. Electrical appliances must meet UL standards. Single-use articles shall not allow migration of deleterious substances or impart colors, odors, or tastes to food. Multi-use food contact surfaces must be smooth, free of breaks, chips, and similar imperfections; free of sharp internal angles, corners, and crevices; finished to have smooth welds and joints; be accessible for cleaning and inspection. Unless specified, wood and wood wicker may not be used as a food-contact surface. Unless specified, copper and copper alloys such as brass may not be used in contact with a food that has a pH below six or for a fitting between a backflow preventer and a carbonator. Beverage tubing and cold-plate beverage cooling devices cannot be installed in contact with stored ice intended to be used as food. Equipment that is not movable must be installed to allow for cleaning in place. Cutting or piercing parts of can openers must be readily removable. The fly strip is not approved for use in a food facility, please remove. Observed wet mops and other cleaning supplies stored in employee bathroom, which may be contributing to the gnat problem. Hang up all mops so they can air dry, dump all wastewater from mop buckets in the sanitary sewer, clean regularly and keep surfaces/drains clean and dry at all times. Increase pest control efforts if necessary. Observed grease accumulation on grease container and surrounding floors. Maintain grease container and surrounding area clean to prevent vermin attraction.

Conditions Observed: Observed open bulk bag of flour on shelf. Once an open bulk bag of food is opened, transfer it to a rigid container with a tight fitting lid to protect foods from contamination. Observed raw eggs stored over ready-to-eat foods in walk-in refrigerator. Store raw potentially hazardous foods under ready-to-eat foods based on final internal cooking temperatures to protect ready-to-eat foods from cross-contamination.

43. Toilet facilities: properly constructed, supplied, cleaned.

Conditions Observed: Observed restroom doors are lacking self-closing doors. Install self-closing mechanism on restroom doors to prevent passage of flies, dust or odors.

ROACHY SAYS:

“Toilet facilities shall be maintained and kept clean. Toilet rooms shall be separated by a well-fitting, self-closing door. Toilet tissue shall be provided in a permanently installed dispenser at each toilet. (114250, 114250.1, 114276)”

44. Premises; personal/cleaning items; vermin-proofing.

Conditions Observed: Observed employee food stored on top of bulk french fry container. An employee shall eat only in designated areas where contamination of nonprepackaged food; clean equipment, utensils & linens, or other items needed protection cannot result. Designate another area for employees to consume food. Observed gnats and fly strips in employee bathroom. Consider installing insect electrocution devices that retain the insect bodies, and hang away from food and utensil handling and storage areas. Recommend contacting professional pest control service to help with fly problem. Observed 3 compartment sink is pulling away from the wall. Caulk sink to wall to provide a smooth surface.

ROACHY SAYS:

“All premises of a food facility shall be kept clean, fully operative, and in good repair. The premise of a food facility shall be free of litter and items unnecessary to the operation of the facility. Areas designated for employees to eat and drink shall be located so that food, clean equipent and utensils, linens, and single-use articles are protected from contamination. Personal items shall be stored in a designated area to prevent contamination of food or utensils. Food prep sinks, warewashing sinks, and handwashing sinks shall not be used for the cleaning or maintenance of tools or cleaning supplies or to dispose of mop water and similar liquid wastes. A dedicated janitorial sink with a drain shall be provided and conveniently located. A separate room, area, or cabinet shall be provided for cleaning supplies. The facility shall be kept vermin proof. Insect electrocution devices shall retain the insect within the device and shall not be located over food or utensil handling areas. 45. Floor, walls, and ceilings: built, maintained, clean.

Conditions Observed: Observed floors are failing throughout the facility. Replace floors to provide a smooth, durable, nonabsorbent surface that is easy to clean. Per permit holder, the tenants and landlord are currently negotiating a new floor. Submit spec sheets or floor samples to this office for approval prior to purchase and installation.

ROACHY SAYS:

“Food facilities shall be fully enclosed in a building consisting of permanent floors, walls, and an overhead structure that meets minimum standards. The walls and ceiling shall be smooth, durable, nonabsorbent, and easily cleanable. Flooring and base coving shall be smooth, durable, and made of approved nonabsorbent material that is easily cleanable. Floor surfaces shall be coved at the juncture of the floor and wall with a 3/8 inch minimum radius and shall extend up the wall at least 4 inches.
