07. Adequate handwash facilities supplied and accessible.

Conditions Observed: Observed soap dispenser in men’s restroom is broken. Repair soap dispenser in men’s restroom as soon as possible and maintain stocked at all times for proper handwashing.

ROACHY SAYS:

“Handwashing facilities shall be provided within or adjacent to toilet rooms, food preparation, and warewashing areas. Handwashing facilities shall be clean, unobstructed, and accessible at all times. Handwashing facilities shall be provided with soap and sanitary single use towels in approved dispensers. 45. Floor, walls, and ceilings: built, maintained, clean.

Conditions Observed: Observed food debris and grease accumulation on cookline floors, under equipment. Maintain floors clean and free of debris. Observed floor is failing in back stock room, in front of produce refrigerator and on the cookline. Repair/replace floor to provide a surface that is smooth, durable, nonabsorbent and easy to clean.

“Food facilities shall be fully enclosed in a building consisting of permanent floors, walls, and an overhead structure that meets minimum standards. The walls and ceiling shall be smooth, durable, nonabsorbent, and easily cleanable. Flooring and base coving shall be smooth, durable, and made of approved nonabsorbent material that is easily cleanable. Floor surfaces shall be coved at the juncture of the floor and wall with a 3/8 inch minimum radius and shall extend up the wall at least 4 inches. Conditions Observed: Observed handwash station in warewash room lacking pump soap and paper towels. Keep required supplies stocked in dispensers at all times for proper handwashing.

08. Proper hot and cold holding temperatures.

Conditions Observed: Measured sliced pineapple 68.5F with probe thermometer, held out of temperature control in garnish tray behind bar. Slice pineapple is a potentially hazardous food that must be held at or below 41F at all times. Server discarded one piece of sliced pineapple, refreshed the ice bath, and added cold pineapple from the under counter refrigerator. Thank you for correcting this major violation. Measured lemon butter 44.5F with probe thermometer in Delfield prep table, bottom portion. Adjust/repair/replace Delfield prep table to hold cold potentially hazardous foods at or below 41F at all times.

ROACHY SAYS:

“Except during preparation, cooking, cooling, transportation to or from a retail food facility for a period of less than thirty minutes, when time is used as the public health control, or as otherwise specified, potentially hazardous foods shall be maintained at or below 41F or at or above 135F. The following foods may be held at or below 45F: raw shell eggs, unshucked live molluscan shellfish, pasteurized milk and pasteurized milk products in original sealed containers, potentially hazardous foods held during transportation. Potentially hazardous foods held for dispensing in serving lines and salad bars may be held at 45F, not to exceed 12 hours in a 24 hour period, if unused portions are discarded. Food preparation shall not exceed two cumulative hours without a return to the required holding temperatures. (113996, 113998, 114037, 114343(a))”

28. Food separated and protected.

Conditions Observed: Observed raw breaded shrimp and raw eggs stored over ready-to-eat foods in Delfield prep table, bottom portion. Store raw potentially hazardous foods under ready-to-eat foods based on final internal cooking temperatures to prevent cross-contamination.

Food preparation shall be conducted wthin an approved food compartment or fully enclosed food facility. Open-air barbecues shall be separated from public access. Conditions Observed: Observed in vinyl flooring in back storage room in front of Arctic Air produce refrigerator. Repair/replace floor to provide a smooth surface that is nonabsorbent and easy to clean. Submit floor samples or manufacturer specification sheets to this office for approval prior to purchase and installation unless you replace it with the same floor.

Conditions Observed: Measured sliced pineapple held out of temperature control 59F with probe thermometer in garnish tray behind bar. Sliced pineapple is a potentially hazardous food that shall be held at or below 41F at all times. Server provided an ice bath for the pineapple. Thank you for correcting this major violation. Measured shredded cheese 47F with probe thermometer in cold table top portion. Observed shredded cheese in a plastic container on top of inserts, not in direct contact with refrigeration. Do not stack inserts to prevent cross-contamination and to prevent temperature violations. Measured chicken tempura batter 45F, fish tempura batter 43F, crushed garlic 45.5F and chopped lettuce 44.5F with probe thermometer in Delfield prep table on the cookline. Adjust/repair/replace Delfield prep table to hold cold potentially hazardous foods at or below 41F at all times.

24. No rodents, insects, birds, or animals.

Conditions Observed: Observed rodent feces adjacent to water heater. Follow CDC guidelines on the handout that was provided for safely cleaning and sanitizing rodent feces. Monitor impacted area for new evidence of rodent activity and increase pest control efforts if necessary. This is a major violation that will require a reinspection on or after 5/13/19.

ROACHY SAYS:

“A food facility shall be kept free of vermin. Live animals are prohibited from food facilities except as specified in 114259.5. Food facility employees shall not care for or handle animals that may be present in the food facility. Food facility employees are required to wash their hands after handling or caring for fish in an aquarium and/or shellfish, or crustacea, in display tanks and as specified in 114259.4. (114259.1, 114259.4, 114259.5)”

28. Food separated and protected.

Conditions Observed: Observed insert of sliced tomatoes stored on top of uncovered cabbage in cold table. Do not stacking inserts to prevent cross contamination and potential temperature violations. Observed open bulk bags of salt, flour and brown sugar on wire rack in dry storage room. Once a bulk bag of food is open, transfer it to a rigid container with a tight fitting lid to protect from contamination and potential vermin attraction. Observed fish food stored in ice cream freezer. Separate fish food from food used for human consumption to prevent contamination. Recommend obtaining a container with a lid for fish food storage. Observed fish stored in direct contact with plastic garbage bags in chest freezer. Obtain food grade bags or containers for food storage to prevent contamination or adulteration of foods. Observed bags of raw chicken and raw pork ribs commingled in a bus tub inside Ascend 2 door refrigerator. Separate raw potentially hazardous foods based on final internal cooking temperatures to prevent cross-contamination.

Conditions Observed: Observed unlabeled chemical spray bottle in bus tub under drip coffee maker. Label all chemicals when removed from original manufacturer’s packaging for safety.

ROACHY SAYS:

“Only those insecticides, rodenticides, and other pesticides that are necessary, bear a legible manufacturer's label, and are specifically approved for use in a food facility may be used in a food facility in accordance with the manufacturer's instructions. Containers used for storing poisonous or toxic materiails such as cleaners and sanitizers taken from bulk supplies shall be clearly and individually identified with the common name of the material. Poisonous or toxic materials shall be stored or displayed to prevent contamination of food, equipment, utensils, linens, and single-use articles. (114254, 114254.1, 114254.2)”

36. Equipment / Utensils approved; installed, clean, good repair, capacity.

Conditions Observed: Observed mold like substance in ice machine interior. Clean and sanitize ice machine per manufacturer’s instructions and keep this piece of equipment on a regular maintenance schedule to prevent mold growth. Observed pooled water and mold like substance in keg refrigerator interior. Service keg refrigerator to find source of pooling water. Clean and sanitize interior of keg refrigerator to prevent mold growth. Observed rubber spatula hanging on wire rack is past its service life. Remove this piece of equipment from service to prevent contamination or adulteration of foods.

All new and replacement equipment shall be properly installed and be certified or classified for sanitation by an ANSI accredited certification program. Electrical appliances must meet UL standards. Single-use articles shall not allow migration of deleterious substances or impart colors, odors, or tastes to food. Multi-use food contact surfaces must be smooth, free of breaks, chips, and similar imperfections; free of sharp internal angles, corners, and crevices; finished to have smooth welds and joints; be accessible for cleaning and inspection. Unless specified, wood and wood wicker may not be used as a food-contact surface. Unless specified, copper and copper alloys such as brass may not be used in contact with a food that has a pH below six or for a fitting between a backflow preventer and a carbonator. Beverage tubing and cold-plate beverage cooling devices cannot be installed in contact with stored ice intended to be used as food. Equipment that is not movable must be installed to allow for cleaning in place. Cutting or piercing parts of can openers must be readily removable. Facilities that wash, rinse, soak, thaw, or similarly prepare foods shall be provided with a approved food preparation sink. Reservoirs that supply water to produce foggers must be cleaned and maintained. Surfaces such as cutting blocks and boards that are subject to scratching and scoring shall be resurfaced or replaced if they can no longer be effectively cleaned and sanitized. 37. Equipment, utensils and linens: storage and use.

Conditions Observed: Observed rice utensils stored in room temperature water. You may store the rice utensil in running water of sufficient velocity to flush particulates to the drain, or in a container of water if the water is maintained at a temperature of at least 135F and the container is cleaned at least every 24 hours or at a frequency necessary to preclude the accumulation of soil residues. Recommend obtaining an electric dipper well that holds utensils in hot water measuring 135F or above.

ROACHY SAYS:

Extra preset tableware shall be removed when a consumer is seated or cleaned and sanitized before further use. Pressurized cylinders shall be securely fastened to rigid structure. 39. Thermometers provided and accurate.

Conditions Observed: Thermometers are missing or broken in the following refrigeration units: -BevAir under counter refrigerator -Delfield prep table -Ascend 2 door refrigerator Provide readily visible thermometers in the warmest part of all refrigerator units (top front).

ROACHY SAYS:

“An accurate easily readable metal probe thermometer suitable for measuring temperature of food shall be available to the food handler. Thermometers shall be calibrated in accordance with manufacturer's specifications as necessary to ensure their accuracy. A thermometer +/- 2 degrees F shall be provided for all refrigerators, hot and cold holding units, and high temperature warewashing machines. (114157, 114159)”

Conditions Observed: Observed the following equipment is visibly soiled: -Ascend 2 door under counter freezer interior -Arctic Air produce refrigerator interior -bulk food bins on cookline exterior -chest freezer in back storage room exterior Clean non-food contact surfaces of equipment regularly.

Observed pooled liquid on bottom of same prep table. Observed pooled liquid on bottom of Arctic Air meat refrigerator. Equipment and utensils shall be kept fully operative and in good repair, repair/replace refrigeration units.
