34. Non-food-contact surfaces clean.

Conditions Observed: Observed minor black speckling on upper left interior of the ice machine at the bar. Clean and sanitize; maintain tidy surfaces.

ROACHY SAYS:

“Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris. (114115(b))”

40. Wiping cloths: properly used and stored.

Conditions Observed: Measured QT in sani-bucket at the server’s station to less than 200 PPM. Keep solution at 200 PPM QT.

ROACHY SAYS:

“Wiping cloths used to clean food spills shall be used only once or stored in a sanitizing solution of an approved concentration as specified in section 114099.6. Dry or wet cloths that are used with raw foods of animal origin shall be kept separate from cloths used for other purposes, cloths that come into contact with raw foods of animal origin shall be kept in a separate sanitizing solution. Wiping cloths shall be free of food debris and visible soil. Working containers of sanitizing solutions for storage of in-use wiping cloths shall be used in a manner to prevent contamination of food, equipment, utensils, linens, or single-use articles. Wet wiping cloths shall be laundered daily. Dry wiping cloths shall be laundered as necessary to prevent contamination of food and clean serving utensils. (114135, 114185.1, 114185.3(d-e))”

43. Toilet facilities: properly constructed, supplied, cleaned.

Conditions Observed: Observed both bathroom doors to be ajar today. Repair doors to be self-closing.

ROACHY SAYS:

“Toilet facilities shall be maintained and kept clean. Toilet rooms shall be separated by a well-fitting, self-closing door. Toilet tissue shall be provided in a permanently installed dispenser at each toilet. (114250, 114250.1, 114276)”

Routine Inspection
45. Floor, walls, and ceilings: built, maintained, clean.

Conditions Observed: Observed flooring/coving has failed the area in front of the mop sink and as one approaches the walkin. Observed 2 to 6 inch gaps between wall and coving where covng has failed. Observed floor sinks downward when stepped upon. Observed approximately 8 by 12 inch hole in wall to the right of the walkin. Discussed potential egress, harborage. Repair/replace floors and wall to be smooth, durable, and non-absorbent. Coving shall conform to CalCode requirements.

“Food facilities shall be fully enclosed in a building consisting of permanent floors, walls, and an overhead structure that meets minimum standards. The walls and ceiling shall be smooth, durable, nonabsorbent, and easily cleanable. Flooring and base coving shall be smooth, durable, and made of approved nonabsorbent material that is easily cleanable. Floor surfaces shall be coved at the juncture of the floor and wall with a 3/8 inch minimum radius and shall extend up the wall at least 4 inches. Routine Inspection
40. Wiping cloths: properly used and stored.

Conditions Observed: Measured 0 PPM QT in several sani-buckets today. Observed an approximately 2 inch bubble in the sanitizer line. Showed bubble to the Owner, who primed the line. Measured QT to be less than 200 PPM; owner requested service and continued to prime the line. Adjust/service/repair to deliver QT at 200 PPM.

22. Hot and Cold Water Available

Conditions Observed: Measured hot water today to be 108 F. Operator adjusted setting on water heater; discussed trouble-shooting. Received proof of correction via email around noon today (9/18/2020). Operator measured hot water to be 123 F. COS. Thank you.

ROACHY SAYS:

“An adequate, protected, pressurized, approved potable supply of hot and cold water shall be provided at all times. Hot water shall be supplied at a minimum temperature of 120F when measured from the faucet. The temperature of water provided to handwashing facilities shall be at least 100F. Handwashing facilities equipped with non-adjustable faucets shall have their temperature set between 100F and 108F. Hot water shall be provided at the food preparation sink and janitorial sink. 07. Adequate handwash facilities supplied and accessible.

Conditions Observed: No paper towels at bar sink. Observed insufficient volume of soap in men’s room dispenser. Staff provided paper towels and refilled the soap dispenser - COS. Keep all handwashing sinks stocked liquid pump soap and paper towels at all times to facilitate frequent, hygienic handwashing.

“Handwashing facilities shall be provided within or adjacent to toilet rooms, food preparation, and warewashing areas. Handwashing facilities shall be clean, unobstructed, and accessible at all times. Handwashing facilities shall be provided with soap and sanitary single use towels in approved dispensers. 29. Washing fruits and vegetables.

Conditions Observed: Observed employee slicing scallions prior to washing scallions. Wash all whole produce in a clean food prep sink prior to slicing.

ROACHY SAYS:

“Produce shall be thoroughly washed in potable water to remove soil and other contaminants before being cut, combined with other ingredients, cooked, served, or offered for human consumption. Chemicals used to wash or peel produce shall meet the requirements specifice in 21 CFR 173.315. (113992)”

45. Floor, walls, and ceilings: built, maintained, clean.

Conditions Observed: Coving and flooring by step to walkin have failed. Repair flooring to be smooth, durable, and free of harborage.

Conditions Observed: No food cards available for review. Keep current California Food Handler Cards on file at the facility. Be able to make file available to EH upon request.

ROACHY SAYS:

“Food facilities that prepare, handle, or serve non-prepackaged potentially hazardous food, shall have an owner or employee who has passed an aproved food safety certification examination. Food handlers that prepare, handle, or serve non-prepackaged potentially hazardous food, shall obtain a valid Food Handler Card within 30 days after date of hire. (113947.1-113947.5, 113948)”

08. Proper hot and cold holding temperatures.

Conditions Observed: Observed fruit purees on counter with no temperature control. Label for mango puree reads “refrigerate after opening”. Per Employee, purees were set out around noon. Keep purees at or below 41 F. Submerge containers in ice if cold-holding with an ice bath.

ROACHY SAYS:

“Except during preparation, cooking, cooling, transportation to or from a retail food facility for a period of less than thirty minutes, when time is used as the public health control, or as otherwise specified, potentially hazardous foods shall be maintained at or below 41F or at or above 135F. The following foods may be held at or below 45F: raw shell eggs, unshucked live molluscan shellfish, pasteurized milk and pasteurized milk products in original sealed containers, potentially hazardous foods held during transportation. Potentially hazardous foods held for dispensing in serving lines and salad bars may be held at 45F, not to exceed 12 hours in a 24 hour period, if unused portions are discarded. Food preparation shall not exceed two cumulative hours without a return to the required holding temperatures. (113996, 113998, 114037, 114343(a))”

20. Consumer advisory provided for raw or undercooked foods.

Conditions Observed: Consumer advisory language is correct yet lacks an asterisk (*) to denote which foods may be served undercooked. Revise menu/advisory to clearly identify which foods may be served undercooked.

ROACHY SAYS:

“If a food of animal origin is served raw, undercooked, or without otherwise being processed to eliminate pathogens, either in ready-to-eat form or as an ingredient in another ready-to-eat food, the permitholder shall inform consumers of the significantly increased risk of consuming those foods by way of a disclosure pursuant to subdivision (b) of 114093 and reminder pursuant to subdivision (c) of 114093, using brochures, deli case or menu advisories, label statements, table tents, placards, or other effective written means. Conditions Observed: Observed bathroom doors to be open at time of inspection. Repair doors to be self-closing and/or post door “keep closed”.

Conditions Observed: Measured foods in the steam table to be below 135 F (black beans, 128 F; veggie gravy, 118.5 F). Per Cook, steam table was turn off about 10 minutes prior (service ends at 2). Beans and gravy were put in the fridge - COS. Adjust/service/repair to hot hold all foods at 135 F or greater. Measured foods in the top of the deli prep to be above 41 F (cheese, 48 F; salmon, 44 F). Adjust/service/repair to cold-hold at 41 F or below.

Conditions Observed: Observed fruit cups in 3door fridge were not covered. Cover foods to prevent contamination.

ROACHY SAYS:

“Food shall be protected from contamination during transportation, storage, preparation, holding, and display. Adequate and suitable counter space shall be provided for all food preparation. Food preparation shall be conducted wthin an approved food compartment or fully enclosed food facility. Open-air barbecues shall be separated from public access. 36. Equipment / Utensils approved; installed, clean, good repair, capacity.

Conditions Observed: Observed lids on the right deli prep in the kitchen to be in disrepair. Repair or replace to be properly fitting. Keep all equipment in good condition.

ROACHY SAYS:

“Utensils and equipment shall be kept fully operative and in good repair. All new and replacement equipment shall be properly installed and be certified or classified for sanitation by an ANSI accredited certification program. Electrical appliances must meet UL standards. Single-use articles shall not allow migration of deleterious substances or impart colors, odors, or tastes to food. Multi-use food contact surfaces must be smooth, free of breaks, chips, and similar imperfections; free of sharp internal angles, corners, and crevices; finished to have smooth welds and joints; be accessible for cleaning and inspection. Unless specified, wood and wood wicker may not be used as a food-contact surface. Unless specified, copper and copper alloys such as brass may not be used in contact with a food that has a pH below six or for a fitting between a backflow preventer and a carbonator. Beverage tubing and cold-plate beverage cooling devices cannot be installed in contact with stored ice intended to be used as food. Equipment that is not movable must be installed to allow for cleaning in place. Cutting or piercing parts of can openers must be readily removable. Facilities that wash, rinse, soak, thaw, or similarly prepare foods shall be provided with a approved food preparation sink. Reservoirs that supply water to produce foggers must be cleaned and maintained. Conditions Observed: Observed excessive puddling by water heater/handwash sink/freezer. Identify source of moisture and correct.

ROACHY SAYS:

“Steam tables, ice machines, ice bins, food preparation sinks, display cases, walk-in refrigeration units, and other similar equipment that discharge liquid waste shall drain into a floor sink with an air gap or other approved type of receptor. All plumbing and plumbing fixtures shall be installed in compliance with applicable local plumbing ordinances, maintained so as to prevent any contamination, kept clean, fully operative, and in good repair. The potable water supply shall be protected with a backflow or back siphonage protection device. Any hose used for potable water shall be of approved materials, labeled, properly stored, and used for no other purpose. Conditions Observed: No paper towels at bar handwash sink. No paper towels at grill handwash; observed soap to be nearly empty at time of inspection. Keep all handwashing sinks stocked with a pump soap and paper towels at all times for hygienic handwashing. Train all staff to promptly restock hand sinks.

Conditions Observed: Observed debris in pooled water in ice scoop container. Operator sent container to dishwasher - COS. Keep all utensils in a clean and sanitized container.

ROACHY SAYS:

“Food-contact surfaces and utensils shall be clean to sight and touch. Food-contact surfaces and multiservice utensils shall be effectively washed, rinsed, and sanitized to remove or completely loosen soils by use of manual or mechanical methods. Precleaning may be required. Manual sanitization shall be accomplished during the final rinse by contact with solution containing 100 ppm available chlorine, 200 ppm quaternary ammonium, or another approved sanitizer. Mechanical sanitization shall be accomplished in the final rinse by achieving a utensil surface temperature of 160F or by contact with solution containing 50 ppm available chlorine, 200 ppm quaternary ammonium, or another approved sanitizer. After cleaning and sanitizing, equipment and utensils shall be air dried. Mechancial warewash machines must be approved and installed and operated according to manufacturer's specifications. Food contact surfaces, utensils, and equipment shall be cleaned and sanitized at the following times: before each use with different type of raw food of animal origin, when changing from raw food to ready to eat food, between uses with raw produce and potentially hazardous food, before using a thermometer, and any time during the operation when contamination may have occurred. Warewash sinks used to wash wiping cloths, wash produce, or thaw food must be cleaned and sanitized before and after these uses. Equipment, food contact surfaces, and utensils must be cleaned and sanitized throughout the day at least every four hours or as needed to prevent contamination. Conditions Observed: Facility continues to lack a consumer advisory. As noted on the previous inspection report, update your menus to include a consumer advisory that includes a disclosure and a reminder. Provide DEH with a copy of your revised menu by 2/28/19.
