Conditions Observed: Measured minor temperature violations in the left prep unit today (chopped tomato, 43 F, sliced melon, 46 F). Adjust/service/repair to cold-hold foods at 41 F or below.

Conditions Observed: Observed excessive food debris on the interior of the microwave at the server’s station. Clean and sanitize; maintain surfaces.

Conditions Observed: Measured foods in the left steam table to be below 135 F (ground beef, 112 F; beans, 119 F; refried beans, 122.8 F). Foods were pulled from the steam table to be discarded. Discussed reheating for hot holding (foods are heated on the flat top). Owner adjusted unit and added more water to steam table. Revise procedure to hot hold foods at 135 F or greater. See related violation. This violation requires a reinspection on/after 7/1/2021.

12. Proper reheating procedures for hot holding.

Conditions Observed: Measured fresh container of cheese sauce in the left steam table to 123 F. As discussed during the inspection, thoroughly cook foods to the specified temperature and hold at 135 F or greater. Use a stem thermometer to measure the core temperatures of food. This violation requires a reinspection on/after 7/1/2021.

ROACHY SAYS:

“Potentinally hazardous food that is cooked, cooled, and reheated for hot holding shall be heated to a temperature of 165F for 15 seconds. Cooked and refrigerated food that is prepared for immediate service in response to an individual consumer order may be served at any temperature. Reheating for hot holding shall be done rapidly, and the time between 41F and 165F shall not exceed two hours. Potentinally hazardous food reheated in a microwave oven for hot holding shall be rotated or stirred midway during cooking, covered to retain surface moisture, be reheated to 165F in all parts of the food, and stand covered for at least two minutes after reheating. Ready-to-eat food taken from a commercailly processed hermetically sealed container or from an intact package from a food processing plant shall be heated to a temperature of 135F for hot holding. 22. Hot and Cold Water Available

Conditions Observed: Observed a reduced flow of water from the hot water tap at the kitchen handwashing sink. Measured water to be 74 F. Showed observation to Owner. Adjust/service/repair to warm water at 100 to 108 F.

“An adequate, protected, pressurized, approved potable supply of hot and cold water shall be provided at all times. Hot water shall be supplied at a minimum temperature of 120F when measured from the faucet. The temperature of water provided to handwashing facilities shall be at least 100F. Handwashing facilities equipped with non-adjustable faucets shall have their temperature set between 100F and 108F. Hot water shall be provided at the food preparation sink and janitorial sink. 28. Food separated and protected.

Conditions Observed: Observed french toast batter above an open container of lettuce. Owner moved batter to lower shelf - COS. Store egg washes, batter with raw egg below ready-to-eat foods. Keep lettuce covered.

Conditions Observed: Observed scoop for potatoes resting on top of food. Owner brought scoop to warewash - COS. Keep handle up and out food.

“Clean equipment, utensils, linens, and single-use articles shall be stored at least six inches above the floor in an approved location within the fully enclosed permitted food facility. Clean equipment, utensils, linens, and single-use articles shall not be exposed to splash, dust, vermin, or other forms of contamination. Non-food items shall be stored and displayed separate from food and food-contact surfaces. During pauses in food preparation utensils shall be stored with handles above the top of the food, on a clean sanitized surface, in running water, or in water that is at least 135øF. Single-use articles and multiservice utensils shall be handled, displayed, and dispensed so that contamination of food and lip-contact surfaces is prevented. Extra preset tableware shall be removed when a consumer is seated or cleaned and sanitized before further use. Pressurized cylinders shall be securely fastened to rigid structure. Linens must be free of food residue and soil and be laundered as required. 06. Hands clean and properly washed; gloves used properly.

Conditions Observed: Observed Cook using a finger cot failed to don gloves. Informed Owner of glove use requirements (see CalCode 113973(a); 113975) and instructed cook to wear a glove over finger cot. COS. Review glove use requirements as described in CalCode with all Cooks. Observed Cook plate onion rings using their barehands. Use a clean and sanitized utensil to onions rings to plates.

Conditions Observed: Observed Cook pound raw, portioned hamburger on deli prep cutting board before placing on grill. Observed ready-to-eat toppings (lettuce, tomato, onion) resting on same cutting board while burger was on the grill. Inspector instructed to Cook to discard toppings; second Cook instructed employee to place toppings in a clean container. Informed Cook to avoid placing ready-to-eat on cutting boards used for raw hamburger. Shared observations with Owner, who will re-train employee. Specify cutting boards and/or utensils to be use solely for raw beef. Train employees to prevent cross-contamination and unintentional adulteration. Please provide proof of training to your Inspector by 4 PM on Thursday, October 15th.

Conditions Observed: Observed Cook pick up spatula from the floor, bring to dish pit, and then fail to wash their hands. Wash hands after picking up utensils from the floor. Train all staff in CalCode handwashing requirements. Contact your Inspection with questions and/or to obtain training materials. Provide proof of training to this office by 11/30/19 to avoid a reinspection.

Conditions Observed: Observed food debris on metal insert, side of plate. Observed debris, dead fruit fly plate (showed observations to PIC). Remove heavily stained dishes from service. Maintain all utensils to be clean to the sight and clean to the touch. Observed food debris on slicer. Employee removed debris and cleaned - COS. Clean slice immediately after use to avoid build-up. The following violations require a reinspection on/after 11/22/19: Observed rust-like build-up, build-up on prong of table mounted can opener. Remove build-up; clean and sanitize. Measured 0 PPM chlorine in the dishwasher. Adjust/service/repair to deliver 50 PPM chlorine. Wash all dishes via the 3-compartment method until the dishwasher has been serviced.

ROACHY SAYS:

“Food-contact surfaces and utensils shall be clean to sight and touch. Food-contact surfaces and multiservice utensils shall be effectively washed, rinsed, and sanitized to remove or completely loosen soils by use of manual or mechanical methods. Precleaning may be required. Manual sanitization shall be accomplished during the final rinse by contact with solution containing 100 ppm available chlorine, 200 ppm quaternary ammonium, or another approved sanitizer. Mechanical sanitization shall be accomplished in the final rinse by achieving a utensil surface temperature of 160F or by contact with solution containing 50 ppm available chlorine, 200 ppm quaternary ammonium, or another approved sanitizer. After cleaning and sanitizing, equipment and utensils shall be air dried. Mechancial warewash machines must be approved and installed and operated according to manufacturer's specifications. Food contact surfaces, utensils, and equipment shall be cleaned and sanitized at the following times: before each use with different type of raw food of animal origin, when changing from raw food to ready to eat food, between uses with raw produce and potentially hazardous food, before using a thermometer, and any time during the operation when contamination may have occurred. Warewash sinks used to wash wiping cloths, wash produce, or thaw food must be cleaned and sanitized before and after these uses. Equipment, food contact surfaces, and utensils must be cleaned and sanitized throughout the day at least every four hours or as needed to prevent contamination. 24. No rodents, insects, birds, or animals.

Conditions Observed: Observed 8 to 10, 0.5 cm black droppings on bottom shelf in dry storage area. Employee removed droppings and sanitized surfaces - thank you. COS. Use the appropriate PPE (gloves, goggles) when using strong bleach solutions. Observed two flies landing on to-go containers in the kitchen. Increase pest control to remove flies from the facility.

ROACHY SAYS:

“A food facility shall be kept free of vermin. Live animals are prohibited from food facilities except as specified in 114259.5. Food facility employees shall not care for or handle animals that may be present in the food facility. Food facility employees are required to wash their hands after handling or caring for fish in an aquarium and/or shellfish, or crustacea, in display tanks and as specified in 114259.4. Conditions Observed: Observed open box of pancake mix on top of stack in plastic trash bin. Keep open bags/boxes in a food grade container with a tightly fitting lid.

Conditions Observed: Observed boxes of hash brown on the floor of the dry storage area. Store all food (including boxed items) at least 6 inches above the floor.

Food shall be protected from contamination by storing the food in a clean, dry location, where it is not exposed to splash, dust, vermin, or other forms of contamination or adulteration, and at least six inches above the floor. 36. Equipment / Utensils approved; installed, clean, good repair, capacity.

Conditions Observed: Observed the use of a deli container, bowl to portion food. As discussed during the inspection, use a utensil that has a handle. Keep handle up and out of food.

ROACHY SAYS:

“Utensils and equipment shall be kept fully operative and in good repair. All new and replacement equipment shall be properly installed and be certified or classified for sanitation by an ANSI accredited certification program. Electrical appliances must meet UL standards. Single-use articles shall not allow migration of deleterious substances or impart colors, odors, or tastes to food. Multi-use food contact surfaces must be smooth, free of breaks, chips, and similar imperfections; free of sharp internal angles, corners, and crevices; finished to have smooth welds and joints; be accessible for cleaning and inspection. Unless specified, wood and wood wicker may not be used as a food-contact surface. Unless specified, copper and copper alloys such as brass may not be used in contact with a food that has a pH below six or for a fitting between a backflow preventer and a carbonator. Beverage tubing and cold-plate beverage cooling devices cannot be installed in contact with stored ice intended to be used as food. Equipment that is not movable must be installed to allow for cleaning in place. Cutting or piercing parts of can openers must be readily removable. Facilities that wash, rinse, soak, thaw, or similarly prepare foods shall be provided with a approved food preparation sink. Conditions Observed: Unable to locate a thermometer in the right reachin on the cookline. Provide a thermometer for this unit.

“An accurate easily readable metal probe thermometer suitable for measuring temperature of food shall be available to the food handler. Thermometers shall be calibrated in accordance with manufacturer's specifications as necessary to ensure their accuracy. A thermometer +/- 2 degrees F shall be provided for all refrigerators, hot and cold holding units, and high temperature warewashing machines. (114157, 114159)”

41. Plumbing: proper backflow devices.

Conditions Observed: Observed leak at 2-part sink adjacent to dry storage area. Repair. Reduce or eliminate free moisture through out the facility to reduce vermin harborage.

ROACHY SAYS:

“Steam tables, ice machines, ice bins, food preparation sinks, display cases, walk-in refrigeration units, and other similar equipment that discharge liquid waste shall drain into a floor sink with an air gap or other approved type of receptor. All plumbing and plumbing fixtures shall be installed in compliance with applicable local plumbing ordinances, maintained so as to prevent any contamination, kept clean, fully operative, and in good repair. The potable water supply shall be protected with a backflow or back siphonage protection device. Any hose used for potable water shall be of approved materials, labeled, properly stored, and used for no other purpose. Measured chlorine to 50 PPM. COS. Use test strips daily to confirm chlorine is entering the dishwasher. Observed build-up on table mounted can opener. Remove build-up; wash, rinse, and sanitize.

Conditions Observed: Observed utensils used to serve ice-cream in a container of water. Keep utensils in a clean and sanitized container; wash, rinse, and sanitize every four hours.

ROACHY SAYS:

“Clean equipment, utensils, linens, and single-use articles shall be stored at least six inches above the floor in an approved location within the fully enclosed permitted food facility. Clean equipment, utensils, linens, and single-use articles shall not be exposed to splash, dust, vermin, or other forms of contamination. Non-food items shall be stored and displayed separate from food and food-contact surfaces. During pauses in food preparation utensils shall be stored with handles above the top of the food, on a clean sanitized surface, in running water, or in water that is at least 135øF. Single-use articles and multiservice utensils shall be handled, displayed, and dispensed so that contamination of food and lip-contact surfaces is prevented. Extra preset tableware shall be removed when a consumer is seated or cleaned and sanitized before further use. Pressurized cylinders shall be securely fastened to rigid structure. Linens must be free of food residue and soil and be laundered as required. 40. Wiping cloths: properly used and stored.

Conditions Observed: Observed soap/bubbles in sani-bucket. Keep wiping cloths in a sanitizing solution of 100 PPM chlorine. Do not put soap in the sani-bucket.

ROACHY SAYS:

“Wiping cloths used to clean food spills shall be used only once or stored in a sanitizing solution of an approved concentration as specified in section 114099.6. Dry or wet cloths that are used with raw foods of animal origin shall be kept separate from cloths used for other purposes, cloths that come into contact with raw foods of animal origin shall be kept in a separate sanitizing solution. Wiping cloths shall be free of food debris and visible soil. Working containers of sanitizing solutions for storage of in-use wiping cloths shall be used in a manner to prevent contamination of food, equipment, utensils, linens, or single-use articles. Wet wiping cloths shall be laundered daily. Dry wiping cloths shall be laundered as necessary to prevent contamination of food and clean serving utensils. (114135, 114185.1, 114185.3(d-e))”

45. Floor, walls, and ceilings: built, maintained, clean.

Conditions Observed: Observed ceiling and floors in dish room to be in disrepair. Repair surfaces to be smooth, durable, non-absorbent and free of harborage.

ROACHY SAYS:

“Food facilities shall be fully enclosed in a building consisting of permanent floors, walls, and an overhead structure that meets minimum standards. The walls and ceiling shall be smooth, durable, nonabsorbent, and easily cleanable. Flooring and base coving shall be smooth, durable, and made of approved nonabsorbent material that is easily cleanable. Floor surfaces shall be coved at the juncture of the floor and wall with a 3/8 inch minimum radius and shall extend up the wall at least 4 inches. (114143(d), 114266, 114268, 114268.1, 114271, 114272)”

Routine Inspection
08. Proper hot and cold holding temperatures.

Conditions Observed: Measured shell egg kept in an ice bath to be 59 F; Operator discarded remaining eggs. COS. Revise method to cold hold shell eggs at or below 41 F (e.g., submerge container in ice or keep eggs in a fridge at or below 41 F). Observed pancake batter on the counter with no temperature control. Either time label the container to show when the batter has been on the counter for 4 hours and discard remaining batter at that time or keep batter at 41 F or below.

ROACHY SAYS:

“Except during preparation, cooking, cooling, transportation to or from a retail food facility for a period of less than thirty minutes, when time is used as the public health control, or as otherwise specified, potentially hazardous foods shall be maintained at or below 41F or at or above 135F. The following foods may be held at or below 45F: raw shell eggs, unshucked live molluscan shellfish, pasteurized milk and pasteurized milk products in original sealed containers, potentially hazardous foods held during transportation. Potentially hazardous foods held for dispensing in serving lines and salad bars may be held at 45F, not to exceed 12 hours in a 24 hour period, if unused portions are discarded. Food preparation shall not exceed two cumulative hours without a return to the required holding temperatures. (113996, 113998, 114037, 114343(a))”

20. Consumer advisory provided for raw or undercooked foods.

Conditions Observed: Facility continues to lack a complaint Consumer Advisory. Cook all food to the final cooking temperature or post a consumer advisory. Use an asterisk (*) to identify menu items that can be served undercooked. The asterisk is linked to the following statement: Consuming raw or undercooked meats, seafood, shellfish, or eggs may increase your risk of foodborne illness, especially if you dhave certain medical conditions. Please provide a copy of your revised menu to DEH by 2/28/19.

ROACHY SAYS:

“If a food of animal origin is served raw, undercooked, or without otherwise being processed to eliminate pathogens, either in ready-to-eat form or as an ingredient in another ready-to-eat food, the permitholder shall inform consumers of the significantly increased risk of consuming those foods by way of a disclosure pursuant to subdivision (b) of 114093 and reminder pursuant to subdivision (c) of 114093, using brochures, deli case or menu advisories, label statements, table tents, placards, or other effective written means. (114012, 114093)”

34. Non-food-contact surfaces clean.

Conditions Observed: Observed an excessive volume of greasy build-up below grill, on baffles, and above drawers below grill. Remove build-up and dispose of per applicable law; clean routinely to avoid build-up.

ROACHY SAYS:

“Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris. (114115(b))”

36. Equipment / Utensils approved; installed, clean, good repair, capacity.

Conditions Observed: Observed build-up on microwave interiors. Clean. Observed Cook cleaning kitchen microwave - COS.

ROACHY SAYS:

“Utensils and equipment shall be kept fully operative and in good repair. All new and replacement equipment shall be properly installed and be certified or classified for sanitation by an ANSI accredited certification program. Electrical appliances must meet UL standards. Single-use articles shall not allow migration of deleterious substances or impart colors, odors, or tastes to food. Multi-use food contact surfaces must be smooth, free of breaks, chips, and similar imperfections; free of sharp internal angles, corners, and crevices; finished to have smooth welds and joints; be accessible for cleaning and inspection. Unless specified, wood and wood wicker may not be used as a food-contact surface. Unless specified, copper and copper alloys such as brass may not be used in contact with a food that has a pH below six or for a fitting between a backflow preventer and a carbonator. Beverage tubing and cold-plate beverage cooling devices cannot be installed in contact with stored ice intended to be used as food. Equipment that is not movable must be installed to allow for cleaning in place. Cutting or piercing parts of can openers must be readily removable. Facilities that wash, rinse, soak, thaw, or similarly prepare foods shall be provided with a approved food preparation sink. Reservoirs that supply water to produce foggers must be cleaned and maintained. Surfaces such as cutting blocks and boards that are subject to scratching and scoring shall be resurfaced or replaced if they can no longer be effectively cleaned and sanitized.
