Dementia Care Coalition Meeting
The Dementia Care Coalition meets every other month on the second Monday from 12:00pm to 1:30pm at Adult Day Health and Alzheimer’s Services, building B at 1901 California Street, Eureka in the Alzheimer’s Resource Library.
What is the Dementia Care Coalition?
• People who work, care and live with dementia
• We come from all walks of life
• Our common purpose is to understand the best way to help individuals and caregivers live and cope with Alzheimer’s disease and Dementia
Dementia Care Coalition Mission - We envision a community in which people with dementia and their family members get the right information and care at the right time, where medical clinicians identify memory loss early in the process and make appropriate referrals to supportive services, and where people with dementia and their families can continue to be active and engaged community members.
