Redwood Coast PACE 10-year anniversary Mixer!

Redwood Coast PACE – a program of the Humboldt Senior Resource Center (HSRC) – is celebrating its 10th Anniversary during September, National PACE Month.
PACE (Program of All-inclusive Care for the Elderly) is a nationally recognized comprehensive healthcare model that allows adults aged 55 and older who need a nursing-home level of care to remain living independently in their own homes for as long as possible. PACE offers personalized medical and supportive care via an interdisciplinary team of providers and care professionals including physicians, nurses, social workers, physical and occupational therapists, transportation coordinators, registered dietician, recreational specialists, and in-home care services. This team provides individualized care for each participant, with a focus on preventive care and the best possible quality of life. All care coordination is handled by PACE, which serves as both the provider and insurer.
To celebrate a decade of service to our community, Redwood Coast PACE will be hosting an Open House event in Fortuna.
Please join us on September 11 from 4:30 to 7:00 p.m. in Fortuna at 3200 Newburg Road. Light snacks, refreshments, prize raffles and more will be provided.
